**Role**:
The Child Intervention Division (CID) leads Children's Services in the delivery of targeted approaches to support young adults from care as they transition to adulthood, through the Transition to Adulthood Program (TAP) post-intervention programs, which includes the Transition to Adulthood (TAP) and Advancing Futures (AF) programs.

TAP empowers young adults formerly in care to meet their full potential through self-selection of specialized pathways to support their needs. The pathways support educational, career, and employment life goals for young adults who are ready to pursue their aspirations. TAP also provides supportive pathways for clients who require a period of stabilization, focused life skills acquisition, or transition to adult support services programming. AF assists youth and young adults who have been or continue to be in the care of Children's Services to access post-secondary education opportunities as they transition out of care and into adulthood. AF provides social and emotional transitional supports and the funding to support their educational goals as a means of obtaining a meaningful career. Between the TAP/AF programs there are over 5000 youth/young adults who meet the TAP/AF eligibility criteria and over 3000 clients enrolled in both programs.

We are currently looking to fill positions located in Central and Northern Alberta and the Calgary Area. This successful applicants from this competition may be used to fill future vacancies.

The Transition to Adulthood Practitioner is a key professional role within the TAP program, providing engaged, relational, and interactive case management to young adults transitioning out of care. TAP Practitioners will also engage with young adults and their support networks to help support a successful transition into adulthood. TAP Practitioners are instrumental in active transition planning with young adults formerly in care, to ensure they are robustly connected to all areas of their life as they emerge into adulthood. TAP Practitioners have the essential role of engaging a young adult's supportive network, as well as fostering connection with a client's self-identified family, to ensure a robust natural support network is established and maintained, that will meet the client's needs.

All TAP participants will receive a comprehensive array of supports to meet their social, emotional and mental health needs. In addition, young adults will select a primary pathway (Stability, Adult Support Services, Career and Employment and Education (Advancing Futures) that best supports them in achieving independence. TAP Practitioners will be asked to identify which pathway they are most interested in supporting. In most instances TAP Practitioners will be provided with their 1st choice, however this may not always be the case and is also dependant on caseload requirements of clients.

**Role Continued**:
Below is an overview of the pathways:

- **Stability Pathway **- TAP Practitioners will provide supports and services to young adults with trauma/complex needs/multiple barriers that need to achieve a level of stability before they can pursue education and/or employment goals in the future.
- **Adult Services Pathway **- TAP Practitioners will provide supports and services to young adults who are eligible for adult disability services through CSS.
- **Career and Employment Pathway **- TAP Practitioners will provide supports and services to young adults who are focused on exploring different careers and/or seeking employment.
- **Education Pathway **(**Advancing Futures) **- TAP/AF Practitioners will provide supports and services to eligible young adults who are ready for post-secondary, upgrading, and/or training and are looking to acquire a marketable ski
