**Position Title**:Life Share Support Worker
**Position Type**:Reduced Full-Time (32 Hours/Week) Contract (18 months)
**No. of Positions**: 1
**Schedule**:Monday to Friday 8:30am-4:30pm, must be flexible to work evenings, weekends, and statutory holidays to support the 24/7 operations
**Position Start Date**: TBD
**Rate of Pay**:$24.32/hour
Community Living Burlington is a social-profit charitable organization serving the communities across the Halton region since 1955. Our mission is to enrich the quality of life of people with a developmental disability and promote their full and meaningful inclusion in the community.
Through LifeShare, Ontario families and citizens (home providers) can offer a safe and nurturing environment for children and adults who have a developmental disability (home sharers).
LifeShare (also know as Host Family) is a residential alternative option offered to children and adults who have a developmental disability. This option enables families to share their home and provide a supportive environment to the child or adult living within the home. The children or adults living in LifeShare are able to benefit from the support of living with a host family, while developing independence and experiencing the structure of living in an alternative family model. With the help of our home providers and LifeShare team, we encourage our home sharers to live independently as possible in their communities.
- _Key Job Duties and Responsibilities _*
- Adhere to the new requirements for the Host Family Program released by the Ministry of Children Community and Social Services.
- Screen new Respite Providers and Host Families Providers as per the Host Family Directives. This includes screening all people over the age of 18 years old that live in the Host Family home.
- Participate in the annual child services audit and adult services audit as per the Ministry of Children Community and Social Services.
- Work in collaboration with the Director and the Executive Director in regards to transitional aged youth moving into host family.
- Monthly or Bi-monthly home visits to Host Families and regular private visits with participants in the program.
- Liaison between program managers/staff on behalf of participant, provider or natural families, members of support network.
- School service coordination meetings.
- Facilitating annual family reviews to each home provider and respite provider.
- Annual Person Centred Plan meetings with participant, day program staff, natural family members and home providers.
- Facilitate Plan of Care meetings for children in program every 6 months and develop the POC.
- Facilitate meetings, visits for new participant in program; set up with day program, transportation etc.
- Coordinate and develop Service agreements annually.
- Use of a vehicle every day for meetings as well as transportation for participants.
- Liaison between participant, natural family, provider and DSO - SIS interviews, TAY worker, CAS, teachers, other school personnel etc.
- Knowledge in FASD is an asset.
- _Key Requirements _*
- University or College Degree in the Human Services Field such as Developmental Service Worker, Social Service Worker, Bachelor of Arts or Science in the Human Services field.
- Ability to manage multiple priorities in a changing environment.
- An enthusiastic team player that is highly organized, flexible and exudes good judgment and diplomacy.
- Proficient in the use of Microsoft Office software and related media/publishing software.
- Ability to complete Ministry mandated specific documents as per Service Provider Guidelines.
- Demonstrated experience completing detailed paperwork and reporting under set deadlines.
- Transit training experience is an asset.
- Knowledge of the Developmental Services Ontario, the intake process and passport funding is an asset.
- Experience in training and teaching.
- Well-developed interpersonal and communication skills and experience in creating natural supports.
- The ability to prepare all appropriate forms, including computer documents and file notes and adhere to MCCSS Host Family Guidelines, MCCSS Children’s Guidelines, ODSP ES Guidelines and CLB Guidelines.
- Willingness to support individual within their home environment and their community.
- Non-Violent Crisis Intervention (NVCI)Training, First Aid/CPR training are assets.
- Willingness to travel i.e.: must have a valid G or G2 driver's license and access to a vehicle and ability to drive people we support in both programs.
