Bruce Peninsula Hospice (“BPH”) is a volunteer-based organization committed to providing compassionate, non-medical care by trained volunteers for families who choose our in-home care and/or bereavement support. We provide our services on the Bruce Peninsula and surrounding rural area at no cost and work cooperatively with community service and health care providers.

**The Role**:
This contract position seeks to develop an inter-organizational approach to enhancing volunteer navigation for persons living with dementia (PLWD) and their care partner (CP). It is a part of a Nav-CARE research project developed by UBCO and supported by funding from Health Canada.

In accordance with the mission, vision and values of Bruce Peninsula Hospice and the UBCO research mandates, the Coordinator will establish a compassionate communities approach to improve dementia care in the Bruce Peninsula area by increasing volunteer capacity for client support.

**Responsibilities**:
1. To form a core leadership team with representation from Bruce Peninsula Hospice and 2-3 other community agencies that have volunteers that serve PLWD and CP.

2. To establish a Community Advisory Committee (CAC) with 15-20 stakeholders and establish terms of reference for support for the project.

3. To conduct an assessment of the community in relation to dementia-inclusive and compassionate community ideals (research toolkit provided).

4. To develop an inventory of local resources and gaps.

5. To implement training material and education for volunteers from participating organizations. This will include Nav-CARE and other relevant training identified by the CAC.

6. To collaborate with BPH Nav-CARE Coordinator and research partners to match trained Volunteers with PLWD & CP experiencing moderate to late-stage dementia.

7. To develop, implement, and evaluate two community-based interventions designed to increase awareness, improve knowledge, and/or enhance engagement opportunities for PLWD & CP.

8. To develop and implement a communication plan to raise awareness in the community of the work being done.

9. To fulfill evaluation requirements of the research project.

**Qualifications**:

- Outgoing personality and ability to engage with people from multiple community service organizations and backgrounds.
- Passion to improve care for persons living with dementia.
- Willingness to develop an in-depth understanding of the Nav-CARE philosophy and its adaptation to dementia care.
- Ability to organize and work with a diverse advisory committee to move the project forward.
- Able to match trained volunteers with dementia clients/families and support them.
- Excellent active listening skills, communication (verbal and written) interpersonal and relationship management skills. Proven ability to work with others including working with seniors and volunteers.
- A solid understanding of rural community service delivery, a commitment to quality care and confidentiality. Relevant work experience in a related non-profit, healthcare or community service setting.
- Has an understanding of Compassionate Communities model.
- Ability to complete requested project evaluation requirement on time.
- Reside within our service area and have familiarity and knowledge of the Bruce Peninsula.
- Integrity and a healthy sense of humour and fun

**REQUIRED**:

- Computer literacy (Word, Excel, PowerPoint).
- BA in applicable field requiring research skills
- Proven organizational abilities
- Proven ability to educate using training materials
- Experience collaborating with community partners or agencies
- Travel throughout the Bruce Peninsula region is required. A reliable vehicle, class G license and two-million-dollar insurance policy is necessary.
