We are seeking a dedicated and proactive **Client Care Coordinator RPN** to join our team.

In this role, you will be a point of contact for our clients and staff, ensuring their needs are met and fostering strong relationships. You will be responsible for aiding in the day-to-day operations of the company, in close communication with the other coordinators and management.

**Responsibilities**
- Serve as the main liaison between clients and our care provider teams, ensuring clear communication and understanding of client needs.
- Oversees coordination of scheduling needs and assists other coordinators
- Analyze client feedback and data to identify trends and areas for improvement.
- Participate in staff orientations, interviews and evaluations
- Coordinate and attend home visits for new client admissions and follow ups,
- Organize and participate in delegations of skills for PSWs
- Provide back up shifts in the community for sick calls and book offs
- Organize and chair or co-chair committees
- Participate in policy and procedures review
- On-Call rotation; 1-2 evenings per week and a minimum of 1 weekend a month

**Preferred Requirements**
- Experience with the Palliative and Pediatric population or keen interest
- Minimum 3 years of community care experience
- Strong client service skills with a focus on building relationships and enhancing client satisfaction.
- Previous education for Palliative and Pediatric cares or interest in pursing education (LEAP core and LEAP peds)
- Strong communication skills, both verbal and written, allowing for effective interaction with clients from diverse backgrounds.
- A proactive approach to problem-solving with the ability to work independently as well as part of a team.
- Experience with visit nursing and wound care
- Experience with Alayacare Software preferred
- Valid Drivers licence and reliable transportation to conduct home, school visits
- RPNAO
- CPR Certification
