Responsible to the Assistant Director of Care, the Social Worker provides services to the residents of Golden West Centennial Lodge. The Social Worker functions as a member of the multidisciplinary team. The Social Worker adheres to the Manitoba College of Social Workers regulations, the Resident Bill of Rights and the Mission statement of Golden West Centennial Lodge.

**KEY RESPONSIBILITIES**:
**Counselling**:

- Provide ongoing counselling or other psychosocial interventions to residents and their families and advocates, as required
- Determine the most appropriate resources, internally or in the community, to meet the identified need of the resident and make referrals to appropriate community agencies to other counseling or mental health professionals; acting as a liaison between residents and community agencies, such as Office of the Public Guardian and Trustee, as required
- Monitor and assess the adjustment process of new residents and provide supportive counseling and intervention
- Assist residents and families and/or advocates, when necessary, in discharge planning

**Delivery of Social Services**:

- Promote and facilitate a resident oriented environment, acting as a resident/family advocate, where indicated
- Function as a member of the multidisciplinary team in providing overall resident care and participate in assessment planning and in the development of evaluation of programs/services in response to resident needs
- Provide consultation to staff, community agencies, etc. for assistance with resolving concerns and meeting needs of residents
- Facilitate resident council meetings and participate in Family Council meetings
- Represent the Lodge and The Salvation Army in a positive manner
- Follow infection prevention and control policies and procedures
- Actively participate in the Continuous Quality Improvement/Risk Management Program

**Admission Management**:

- Co-ordinate and maintain the waiting list, in collaboration with WRHA Long Term Care Access Centre
- In conjunction with nursing staff, review and select applicants for admission to vacancies
- Respond to inquiries from people on the waiting list and/or their families

**Administration and Compliance**:

- Model and foster an environment of positive customer service in all aspects of care
- Facilitate communication in complex care issues to foster positive resident outcomes
- Maintain relevant documentation on the resident’s chart
- Schedule, facilitate and participates in multidisciplinary Resident Care Conferences and Admission Conferences
- Ensure that accreditation/Manitoba Health Standards are met or exceeded in service delivery
- Participate in committee functions, as required
- Recommend to immediate supervisor the equipment and supply needs of the department
- Participate in ongoing personal and staff development in conjunction with ADOC
- Model positive working relationships with the management, and co-workers adhering to the Workplace Harassment Discrimination and Violence Prevention policy
- Maintain a safe living and working environment through compliance with policies and procedures, rules and regulations governing facility safety and safety in the workplace

**QUALIFICATIONS AND EDUCATION REQUIREMENTS**:
**Education, Qualifications and Certifications**:

- Completion of a university program in a relevant specialized area consisting of four academic years, e.g., Bachelor of Social Work
- Active member of Manitoba College of Social Workers

**Experience and Skilled Knowledge Requirements**:

- More than 1 year relevant experience
- Specialized knowledge in gerontology and family/representative counseling
- Specialized knowledge in dementia care
- Experience in Long Term Care setting
- Experience in leading resident centred approach
- Knowledge and understanding of Resident Bill of Rights
