**Social Worker**

CIUSSS de l'Ouest-de-l'Île-de-Montréal is a nonprofit community-based organization that provides services to people with mental health issues, substance use disorders, and co-occurring disorders. We provide services in a supportive environment to individuals in recovery.

**Responsibilities**
- Provide counseling and psychotherapy services to clients and families (including substance abuse treatment)
- Collaborate with other service providers to develop individualized treatment plans for clients
- Provide counseling and psychotherapy services to adults, adolescents, and families (including substance abuse treatment)
- Conduct individual assessments of clients
- Document all sessions in order to meet state regulations
- Complete follow up calls with clients after discharge of a treatment plan
- Complete monthly case management reports for client cases
- Attend case conferences as needed (e.g., monthly review of progress reports on client cases)
- Attend professional meetings and trainings as needed
- Maintain current knowledge of relevant research, best practices, and trends in the field of mental health and substance abuse
- Serve as an advocate for clients by participating in advocacy efforts that promote the best interests of clients, families, staff, and community members.

**Salary**: $28.00-$50.00 per hour

Schedule:

- Monday to Friday

Supplemental pay types:

- Overtime pay

**Education**:

- Master's Degree (preferred)
