**Summary of Role Responsibilities**:
In accordance with the Mission, Vision, and Values of St. Michael’s Centre, and reporting to the Chief Executive Officer, the Spiritual Health Practitioner promotes a psychologically safe environment for patients, residents, families, visitors, and staff and works in collaboration with the interdisciplinary teams as assigned. The Spiritual Health Practitioner’s role is to provide comprehensive spiritual/pastoral care, and emotional support to patients/residents/families in a manner that is appropriate to belief systems and cultural diversity.

**Role Responsibilities**:
1. Contributes to the achievement of St. Michael’s Centre and Hospice’s philosophy, goals, objectives, policies, and standards.

2. Provides spiritual, emotional, and discernment at times of anxiety, loss, death, and spiritual or crises.

3. Promotes the on-going spiritual well-being of patients/residents through supportive listening, counseling and prayer, as well as spirituality groups and programs.

4. Attends and participates in program-based care team meetings as needed, including patient/resident care rounds, patient/resident care conferences, and ethical decision-making conferences.

5. Makes referrals to other disciplines, when appropriate, based on the concerns of patients/residents around medical care, nutrition, mobility, and ethical needs.

6. Documents in patient/resident charts spiritual assessments, significant interventions and referrals to ensure caregivers are informed of pastoral care given in response to spiritual/religious and cultural needs.

7. Establishes and maintains liaisons with clergy, faith-specific volunteer visitors, and affiliated denominations to support their programs of visiting, worship, and communion services via phone calls, visits, and/or by attending spiritual programs at houses of worship.

8. Maintains and distributes the emergency clergy list for Long-Term Care and Hospice and contacts chaplains, religious, spiritual, or cultural representatives for patients and residents, when requested.

9. Maintains a monthly schedule of spiritual, pastoral, and religious services.

10. Plans and participates in facility-wide celebrations, such as Worship Services and Memorial Services for individuals or groups, including organizing, coordinating, and leading worship and devotional periods as well as executing special services for occasions such as Easter, Christmas, and Remembrance Day.

11. Serves on, and submits reports to, St. Michael’s Centre’s Spiritual & Ethical Resources Committee, which meets quarterly.

12. Encourages staff awareness of, and respect for diverse expressions of spirituality through in-services and one-on-one conversations. Encourages staff to be aware of the healing that a patient/resident can experience through their beliefs and practices of faith.

13. Acts as a community liaison, including participation (with health authority leaders) in the coordination and supervision of spiritual health volunteers and visitors from various spiritual, religious, and cultural groups. The Practitioner participates in orientation and facilitation of staff, community faith representatives, and volunteers.

14. Performs other related duties as required.

**Qualifications**:
A relevant Master’s Degree (Theology, Pastoral Studies, Spirituality) from an accredited theological institution and at least one advanced unit of Clinical Pastoral Education (CPE), or an equivalent combination of education and experience.

Endorsement by a recognized faith tradition/group and membership with the Canadian Association for Spiritual Care (CASC) are preferred.

- Demonstrated ability to work as a member of a collaborative, interdisciplinary team;
- Ability to work flexible hours;
- Ability to organize and prioritize work and activities;
- Physical ability to perform the duties of the position.

**Job Types**: Part-time, Permanent
Part-time hours: 22.5 per week

**Salary**: $32.00-$34.00 per hour
