** AND SCOPE**

Assists the Community Support Lead and team to help meet client and family needs in relation to the impact of a palliative illness, death, and grief. Assists with community client programs, individual support and counselling and group support and facilitation both in house and in community. Full-Time position on a 7-month contract.

**DUTIES AND RESPONSIBILITIES**

1. Assist with intake services for in-home visiting, palliative care, caregivers, and bereaved individuals.

2. Manage your own case load of community clients, as directed by the Community Support Lead for both group support and one on one counselling support.

3. Work collaboratively with Community Support Lead and other Community Support Workers to facilitate seamless, integrated service to clients.

4. Work collaboratively with external referral agencies and refer to other healthcare professionals and community agencies when appropriate.

5. Ensure client assessments are conducted in a timely manner.

6. Develop care plans and provide supportive services to help meet client / family psycho-social needs.

7. Provide timely and comprehensive information to the Volunteers Coordinator on client/family needs for volunteer matching.

8. Attend case conferences to review client data and to facilitate care and services.

9. Assist with the development and coordination of programs and services. In this respect, work with the Community Support Lead, the Volunteers Coordinator, and trained volunteers.

10. Address complex concerns relating to clients in your care to the Community Support Lead, for additional support where necessary.

11. Interpret the role of hospice to clients and their families.

12. Ensure current information is available on community services and resources.

13. Assist with client special events such as Lights of Love - annual memorial services and special events such as Widows Social.

14. Update InfoAnywhere/Meditech and all necessary and confidential documentation related to clients.

15. Maintain and produce statistical data / reports as required.

16. Provide support and guidance to placement students as part of a team approach to their program needs

Periodic:
17. Attend staff meetings.

18. Support the fundraising efforts of the organization.

19. Assist with general office functions as needed.

20. Provide vacation and other cover for the other Community Support Staff.

**Job Type**: Fixed term contract
Contract length: 7 months

**Salary**: $25.00-$28.00 per hour

- 8 hour shift

- Collingwood, ON: reliably commute or plan to relocate before starting work (required)

**Education**:

- Bachelor's Degree (preferred)
