As a professional, you will assist patients and caregivers, collaborating with healthcare providers to facilitate resources and access. Your role is essential in ensuring a quality experience during treatment and navigating reimbursement processes. Key Responsibilities:
- Serve as primary contact for Patient Support Program patients
- Provide education on therapies and medication adherence
- Coordinate care with physicians and healthcare professionals
- Assist in patient enrollment and onboarding
- Conduct assessments and monitor patient progress
- Active nursing license in Alberta or British Columbia
- Strong communication and organizational skills
- Proficient in Microsoft Office and electronic documentation
- Ability to work 2.5 days per week within program hours
- Willingness to travel to clinics as required
