- Coordinate and supervise Community Support Workers in the field
- Develop effective care plans for Community Health Services
- Monitor compliance with health service regulations
- Promote Community Health Services publicly and positively
- Guide Health and Safety practices actively
- Registered Nurse or Registered Practical Nurse credential
- 3-5 years of relevant experience or equivalent
- Registration with the College of Nurses of Ontario
- Strong computer skills for scheduling and communication
- Valid CPR Certification and background check
