Elevate your career as a Behavioural Support Worker with LOFT Community Services in Toronto, ON. Provide essential support and care to seniors facing mental health and behavioral challenges. This onsite position requires a compassionate individual ready to enhance the quality of life for seniors experiencing responsive behaviors. As part of a multidisciplinary team, you will collaborate closely with the Psychogeriatric Case Manager to develop and implement effective Behavioral Support Plans. This role invites candidates with strong observational skills and a commitment to client-centered care. Key Responsibilities:
- Serve as the primary PSW for assigned clients
- Conduct detailed observations during initial intakes
- Implement Behavioral Support Plans with the team
- Monitor and report on client behavioral changes
- Deliver compassionate, hands-on support in hospitals
- Post-secondary education in Human Services or equivalent
- Knowledge of behavioral support principles
- Experience in crisis intervention and problem-solving
- Valid First Aid & CPR certification
- Driver’s license and vehicle access required
