The Child and Youth Permanency Worker provides support and services to children and youth in care, supporting their integration into family and community. Services are provided from admission to care until readiness to leave care (up to age 23), in accordance with legislative requirements, Ministry Standards, and the Society's strategic direction, values, and best practice framework.

The Child and Youth Permanency Worker (Float) provides support across a variety of Children and Youth Services programs based on operational needs. The incumbent may be assigned to Children in Care, Resource Services, Kinship Services, Adoption and Permanency, or RSG caseloads. While typically assigned to one service area at a time, assignments will change periodically to address staffing and service delivery requirements.

Main duties and responsibilities include, but are not limited to:

Duties are typically assigned from the following service streams to address specific service needs while ensuring a seamless approach to providing care to children and youth in care.

Children / Youth in Care Services:

  • Ensures children and youth in care receive services in accordance with legislative requirements, Quality Standards Framework (QSF), Agency policies, and best practices.
  • Develops, implements, and reviews plans of care and permanency plans that support the safety, wellbeing, and long-term stability of children and youth.
  • Builds and maintains meaningful relationships with children and youth, ensuring their voices are heard and considered in service planning and decision-making.
  • Supports placement stability and quality of care by collaborating with caregivers, families, schools, and community service providers.
  • Facilitates family connections, access arrangements, case conferences, and transition planning to support permanency and positive outcomes.
  • Coordinates services and resources to address children's and youth's developmental, educational, emotional, and life skills needs.
  • Prepares for and participates in court proceedings, providing documentation, recommendations, and evidence in support of permanency planning.

Ready, Set, Go Services for Children and Youth:

  • Supports youth participating in RSG by developing and implementing transition, independence, and service plans in accordance with legislation, directives, and Agency requirements.
  • Builds and maintains supportive relationships with youth, promoting their physical, emotional, educational, social, and life skills development.
  • Assessments needs and connects youth with community resources, supports, and opportunities that foster independence and long-term success.
  • Assists youth with housing, education, employment, family connections, and other aspects of transition planning, while ensuring their voice is central to decision-making.
  • Coordinates services, case conferences, and advocacy efforts to support youth in achieving their goals and successfully transitioning to adulthood.

Resource/Kin-in-Care:

  • Recruits, assesses, and approves kin foster, foster, foster-to-adopt, and adoptive families in accordance with legislative, Ministry, and Agency requirements.
  • Provides education, training, guidance, and ongoing support to prospective and approved resource and kin foster caregivers.
  • Identifies and recommends appropriate placements that support the safety, well-being, development, and permanency needs of children and youth.
  • Manages approved resource and kin-in-care homes, ensuring compliance with licensing requirements, standards, and Agency policies.
  • Collaborates with caregivers, families, and community partners to support placement stability, permanency planning, and successful transitions.
