$53,000.00-$66,000.00 (commensurate with experience)We are seeking a: Supervisor for Early Intervention Hoarding Services.
This is a temporary, full-time position ending June 11, 2027.
Reporting to the Senior Manager, Responsive Community Care, you will oversee a team of volunteers and paid coach(es) that support vulnerable clients with hoarding challenges that put their independent living situation at risk.
The main areas of focus for the Supervisor include training and supervising their team; receiving and processing referrals from community members, service providers, and family members; conducting initial home assessments with all clients and collaboratively customizing care plans; assigning, scheduling, and short-term case management for clients active with program, supporting and monitoring a team of volunteers and hoarding support coach(es); responding to inquiries regarding VHA Services and providing information and referrals.
The Supervisor will also participate in VHA services and quality initiatives and ensure all activities conducted within the policies, procedures and standards of confidentiality of VHA. #LI-CF1Key Areas of Accountability:Receives inquiries for service and conducts initial visits with clients to determine eligibility and collaboratively establish care plan.
Conducts Field Supervision on all clients receiving services and collaboratively supports with Support Group Facilitation.
Supervises, supports and guides Hoarding Coaches and Hoarding Support Volunteers.
Contributes to community support teams and service effectiveness.
Provides program support and administration, including inputting all client data, information relating to scheduling, client care plans, changes to service, and other relevant documentation on electronic client database streams, amongst other items.
Supports service outreach development and marketing.
Required
Qualifications:
A BSW Degree or Social Service Worker Certificate, Psychology, Addictions and Mental Health or equivalent combination of education and experience.
Minimum of 3-5 years of experience working with mental health client populations, specifically with hoarding tendencies.1-3 years of Supervisory experience preferred, with knowledge of trauma-informed care and anti-oppressive practice.
Experience providing case coordination to vulnerable adults with hoarding, mental health, and addictions challenges in home and community setting.
Experience facilitating educational sessions, trainings, client support groups.
Experience teaching and mentoring group of staff and/or volunteers.
Experience with service navigation and identifying gaps in service to promote long-term sustainable outcomes.
Experience supervising staff and volunteers within a unionized setting and working knowledge of relevant employment and health and safety legislation and collective agreements.
Experience in program evaluation, including developing logic models, setting performance indicators, and analyzing outcomes to inform continuous improvement.
Familiarity with reporting to funders and stakeholders using data-driven results to demonstrate impact.
Excellent written skills and ability to write grant proposals, including needs assessments, project planning, and budget development.
Travel in Toronto (North York, York, Scarborough, Etobicoke) is required; a valid driver’s license and access to a vehicle is preferredDemonstrated strength in family and individual work and case management.
Knowledge of community and health services and established contacts with other agencies.
