As part of a multi-disciplinary approach to full-service family medicine, an interested NP would work collaboratively in the team environment and start their own patient panel as per Ministry requirements.This will be a full-time or part-time contract position. This clinic is excited to add to their team a Nurse Practitioner interested in working together to provide primary care for the surrounding community of patients. This initiative is part of Primary Care Networks (PCNs) evolving in Vancouver. It serves the francophone population of the Lower Mainland, aiming to provide primary, low-barrier, culturally safe, and coordinated care. The CHC offers longitudinal care (including mental health and addiction services). The patient population will consist of francophone newcomers, LGBTQ2S+ and Indigenous individuals, isolated seniors, youth, and other groups who have faced barriers accessing healthcare in the English-speaking primary care system. Very diverse population in terms of immigration status, ethnicities, gender, income levels, etc.The clinic will adopt a holistic approach, and will conduct appointments both virtually and in person to promote care accessibility. There are 4 exam rooms and 1 charting room.
