We have been a leader in the delivery of innovative support services for more than 35 years as we work to improve the lives of unhoused and marginalized youth and families in York Region. At 360°kids, we provide a unique continuum of care approach with programs and services that are essential to the health and well-being of youth. These proven strategies help to keep youth safe by providing stable housing options with 360° wraparound supports. The Aftercare & Transition Worker provides one-on-one case management and workshop-based interventions, to provide youth leaving care and 360°kids youth moving from transitional/emergency housing into private market rentals with support in developing independent living skills. Through workshop-based, life skills focused, programming youth will learn tangible life skills required for daily living, self-maintenance and obtaining and sustaining gainful employment. One to one case management will focus on education/employment assistance; finding appropriate, stable, and affordable housing; relationships; life skills; identity; youth engagement and emotional healing. Key Responsibilities Provide support to youth leaving care or youth leaving 360°kids transitional/emergency housing programs to promote interdependence and help the youth develop a system of supports within the community. Provide one to one case management focused on education/employment assistance; finding appropriate, stable, and affordable housing; relationships; life skills; identity; youth engagement and emotional healing. Plan and facilitate life skills programming for the youth in order to foster independence in participants by encouraging positive attitudes and reinforcing concrete daily living skills which are necessary for successful independent living. Assist and provide support in the development of youth action plans. Co-ordinate action plan activities and monitor progress. Provide client focused counselling skills and role modelling in an adaptable and flexible approach taking into consideration the unique qualities of each client. Encourage and support clients to secure suitable housing within the community and assist youth to access various community programs regarding housing, mental health, etc. Maintain active and effective communication with the Manager of Community Housing Programs. With the Manager supports, creates and implements approved strategies and plans that will result in the achievement of the program’s goals and objectives. Report on a regular basis to Program Manager regarding issues of need and/or concern. In the event of a crisis or critical incident, immediately reports to appropriate level. Represents the organization and program as required with various aspects of the community including property management staff, schools and Children’s Aid Society and client family members. Completes all mandatory recording and reporting including daily documentation, statistical data collection and completing registration forms. Provides reports and related information to the Program Manager of Community Housing Program as required. Education and skills required Post-Secondary degree/diploma Child/youth related field (e.g. Child and Youth, Social Service Worker, Social Worker, Psychology, Sociology) or the equivalent combination of education and experience working with youth. Minimum of three years’ experience working with “at-risk” or street involved youth. Current First Aid/CPR certification an asset. Current Broad Record Check (criminal background check). Car and valid driver’s license with a clean driver’s abstract, insurance liability coverage at $2,000,000. Demonstrated extensive knowledge of working with youth, including understanding of the regulatory environment (e.g. CFSA). Demonstrated client focused counselling skills and role modelling. Must possess excellent communication skills and have the ability to be adaptable with a flexible approach taking into consideration the unique qualities of each client. Exceptional interpersonal skills that promote positive team building, negotiation, and conflict resolution. Will possess a broad knowledge of community resources and the ability to strongly advocate for clients and professional referrals. This job posting is for an existing vacancy within our organization.
