This position is ideal for therapists who value flexibility, autonomy, and meaningful client relationships within a community-based model. Please note, this position will require in-person home visits with vulnerable community members.While we thank all interested applicants, we are only able to contact those who we are inviting to an interview.Key ResponsibilitiesProvide follow-up sessions focused on mobility, gait training, strength/balance retraining, pain management, and functional progressionDevelop, implement, and adjust individualized treatment plans in alignment with client goalsEducate clients and caregivers on home exercise programs, safety strategies, functional mobility, and equipment useIdentify and address fall risks, environmental barriers, and safety issues in the homeCollaborate with OTs, PSWs, nurses, physicians, and HCCSS/OH@H care coordinators to support coordinated careComplete documentation and charting accurately and in a timely manner according to community-care standardsMaintain ongoing communication with the office regarding availability, scheduling updates, and urgent visit capacityUphold The Care Company's commitment to safe, ethical, and compassionate community careScheduling & TravelFlexible full-time and part-time (Monday-Friday), occasional Saturday/Sunday availability based on client needs. Travel may be required between client homes.Clients are located across Toronto, York, or Simcoe including but not limited to:AllistonBradfordGeorginaNewmarketAuroraMapleStouffvilleMarkhamAbility to independently manage scheduling, respond promptly to referrals, and coordinate visit times directly with clients.Qualifications & ExperienceHome and community care experience is a MUST.
