OverviewJoin to apply for the Nurse Practitioner - Embrace Clinic role at Fraser Health Authority.Location: Surrey, B.C.Salary: CAD $59.56 - $85.62 per hourAs part of Fraser Health’s Forensic Nursing Service, the Embrace Clinic Nurse Practitioner provides trauma informed, culturally safe medical care to individuals aged 13 and above who have experienced recent violence. The Nurse Practitioner will work closely with community partners such as community services, legal services (policing and courts) and the Ministry of Child and Family Development.Learn more about Embrace Clinic and be part of a clinical enhancement program with in-house skills development and a New Graduate NP Community of Practice to support transition into independent practice.ResponsibilitiesDiagnoses and treats undifferentiated diseases, disorders, and conditions within the Nurse Practitioner scope of practice; writes orders for treatment and medications; provides first line care in emergencies.Monitors ongoing care, orders and interprets screening diagnostic investigations; analyzes information to plan treatment.Establishes priorities for health management; provides follow-up treatment; communicates health findings, diagnoses and priorities with patients and families; supports and counsels patients.Collaborates with physicians and other health/social care providers; develops and implements treatment plans; may admit/discharge patients according to policy.Prescribes drugs within regulatory standards and employer policies.Assigns work to other nursing/health personnel; oversees education and supervision as necessary; participates in hiring and evaluation of staff.Participates in research contributing to improved patient care and population health.Maintains population health focus through screening and health promotion activities for at-risk populations.Participates in interdisciplinary education and fosters health care partnerships.Develops, implements, and evaluates policies and procedures related to nursing and interprofessional care.Performs other duties as required.QualificationsEducation and ExperienceCurrent practicing registration as a Nurse Practitioner with the British Columbia College of Nurses and Midwives (BCCNM).Recent relevant clinical nursing experience.CompetenciesDemonstrates Fraser Health Leadership Framework values: Clear, Caring and Courageous; creates conditions for people to succeed.Professional/Technical CapabilitiesAbility to communicate and collaborate with patients/families about health findings, diagnosis, treatment, self-care and prognosis.Ability to collaborate, consult with and refer patients to physicians and other health professionals when appropriate.Ability to critically assess and evaluate health research to determine best practices; promote education and evidence-based practice.Ability to assess population health trends and implement population-based prevention and health promotion strategies.Ability to implement and evaluate planned change.Ability to supervise others and evaluate care delivered.Ability to lead a team and work within a team.Ability to self-direct and adapt in complex, dynamic situations.Ability to transfer knowledge, teach, coach and mentor others.Ability to identify and respond to legal and ethical issues in care.Ability to self-assess performance and pursue professional development.About Fraser HealthFraser Health is a large health authority serving Metro Vancouver and the Fraser Valley.
