Working with P3 Health Partners By joining our team, you and your patients have access to our local team of experienced professionals, which includes MDs, NPs, and RNs. This position can be part‑time or full time hours. Responsibilities Conduct comprehensive Annual Wellness Visits in compliance with CMS guidelines. Perform health risk assessments and screenings. Review current medications, reconcile lists, and assess adherence. Identify risk factors, gaps in care, and potential health concerns. Provide patient education on preventive health measures and chronic condition management. Document all encounters accurately and timely in the EMR system. Utilize tools to manage clinical and financial resources. Qualifications3> Current, unrestricted Nurse Practitioner license in California Training or interest in geriatrics welcome, but not required. Current DEA license. Board Eligible or Board Certified in Family or Internal Medicine. Excellent communication, documentation, and interpersonal skills.
