As a member of the Mobile Crisis Team, this position is responsible for services for individuals in crisis requiring de-escalation, mental health and risk assessments, and coordination of care and supports in the community. The position also involves working collaboratively with Mobile Crisis Team partner agencies, emergency room departments, community mental health and addictions, and psychiatrists. The Mobile Mental Health Clinician will participate in collaborative care efforts with other interdisciplinary service providers such as, but not limited to, Community Mental Health, Primary Care, Psychiatry, EMS, justice, victims’ services, child and family services, and police services.The Mobile Mental Health Clinician will also answer calls coming in from the Mental Health and Addictions Access Line. The Mobile Mental Health Clinician assists clients, their families, their caregivers, other health care professionals, and community partners from a broad spectrum of health, social, and community programs and services by utilizing information and telecommunications technologies to support the delivery, management, and coordination, of mobile crisis and community-based services using evidence-based, interdisciplinary, and integrated care pathways. Clinicians use telehealth to deliver care, provide education, monitor client progress, access patient records, obtain information, and foster communication with clients, Mobile Crisis Team members, family members, caregivers, health care providers, and community partners.The Mobile Mental Health Clinician provides intervention(s) for acute and chronic mental health care needs, illnesses and injuries, and supports emergent events through the use of telecommunications technologies including: computer, internet, telephone, text messaging, email, radio, tablets, video and audio conferencing, computer information systems, and other digital platforms and emerging health technologies. The Mobile Crisis Telehealth Clinician provides teleconsultation, client crisis and risk assessment, remote monitoring, Mobile Crisis Team support, referral/navigation, case management/follow-up, and other system support services to promote timely, coordinated and integrated crisis and follow-up care for clients; to facilitate improved patient flow through the health care system; to increase client, family, caregiver, healthcare provider, and community partner satisfaction; to facilitate improved communication between health, social, and community care programs and services, including health care providers; and to provide a point of contact and resource for the aforementioned individuals and agencies.**Duties include but are not limited to:*** Staffing Mental Health and Addictions Access Line that is open 24 hours per day, seven (7) days per week, and 365 days per year* Includes working when Mobile Crisis Teams are operating (10:00 a.m. to 10:00 p.m.) as well as not operating (10:00 p.m. to 10:00 a.m.)* Provides immediate response on the crisis line; triages crisis calls; and, after assessment, contacts police, EMS, and/or Mobile Crisis Team if needed* Assesses the potential risk to Mobile Crisis Team members in the outreach environment* Utilizes crisis intervention skills when appropriate to understand the factors contributing to the client’s distress and develops a working case formulation* Uses verbal strategies to engage clients; attempts to de-escalate clients calling in distress; and tries to diffuse emergency/crisis situations* Provides support to caregivers by trying to diffuse conflict between the client and their caregivers* Makes clinical decisions about risk and crises and provides crisis intervention when appropriate* Triages all incoming client calls to assess client health care needs; responds to questions pertaining to health promotion, disease prevention, and chronic disease management; engages in case management and monitoring of existing clients; assists with system navigation; and redirects inquiries and makes referral(s) when necessary and appropriate* Performs comprehensive and focused health assessments, including mental health and risk assessments; health history; and physical assessment, using a holistic framework and multiple assessment techniques relevant to the individual client’s stage of development, education, physical environment, social support networks, culture, health, and health conditions* Initiates a client record and care plan upon initial contact with the client and/or the client’s care provider and facilitates the* Fosters positive relationships and strong partnerships with Mobile Crisis Team members, law enforcement, EMS, hospitals, mental health and social service agencies, medical providers, and other community partners to build an effective delivery of the Mobile Crisis Service in the community* Will provide follow-up check ins, as appropriate, for persons who had been in crisis, resulting in either a telephone intervention or deployment of a Mobile Crisis Team* Educates the community about the functioning and limitations of the Mobile Crisis Service as appropriate* Expected to respond to emergency calls and to address potential crises with Mobile Crisis Team members.* Possesses in depth knowledge of suicidal and self-destructive behaviors as well as homicidal and aggressive behaviors, to include the risk factors associated with the risk of potential harm to self and/or others* Uses substantive knowledge of numerous areas including but not limited to, mental illness, cognitive functioning, medical co-morbidity, and substance abuse to formulate clinical impressions and recommendations* Provides appropriate referrals for community resources that align most closely with the client’s treatment plan* Collaborating with community partners to ensure clients receive timely service to help avoid any unnecessary emergency room visits* Participates in providing staff training, workshops and information sessions* Participates in the development and implementation of administrative and clinical programs as directed by the supervisor* Assists with the monitoring and evaluation of services provided (providing statistics, case reviews and PSMS reports for complaints/compliments)* Ongoing commitment to develop knowledge and skill with emerging treatment and service requirements* Participates in team meetings, staff meetings and regularly scheduled case reviews**Minimum Qualifications:***
