Job Summary We are hiring dynamic, passionate Occupational Therapists (OTs) who want to be part of the transformational change in home and community care by providing care in a truly client-centered approach. You will be part of a diverse care team that will provide services to a roster of clients in the community. You will be part of a collaborative team that will conduct joint care planning with a focus on client-centered goals, addressing the client’s physical, cognitive, social, emotional, and spiritual domains, and creating a holistic care plan. Each client is assigned a primary care clinician, a member of the interdisciplinary team who will lead care planning for that client and facilitate the interdisciplinary team to reach the client’s care goals. As part of the SE Transitions team, you will be accountable for the care and services you provide and the outcomes your clients achieve. Independent Contract or Employment options are available. At SE, you meet your patients where they are, and together you take them to new heights Job Responsibilities Conduct assessments Formulate goals and recommendations Plan and implement client-centered, goal-directed treatment programs Requirements Graduate of an accredited program in Occupational Therapy and in good standing with the College of Occupational Therapists of Ontario Recent experience in managing patients in acute/rehab or community-based rehabilitation programs preferred Reliable, flexible, creative, self-directed professional, able to work effectively and collaboratively with rehab assistants as well as with a high-performance interdisciplinary rehabilitation team Sound knowledge of community resources an asset Effective interpersonal, organizational and communication skills Positive work and attendance record essential Ability to adapt to new computer applications Hybrid work approach: remote and at client sites in acute/rehab or community-based settings Valid driver’s license and vehicle, able to travel About SE Health SE Health is a not‑for‑profit social enterprise advancing health with heart.
