Location Proximity Description of Position The Transitional Support Facilitator (Social Worker) is responsible for providing short term (30-90 days) support to people with a developmental disability or dual diagnosis and their families in crisis. This is provided through Peel Crisis Capacity Network (PCCN) as part of Community Living Mississauga. Working under the guidance of a Coordinator, this position is responsible for ensuring high quality support is provided while maintaining compliance with all applicable regional and provincial laws. Description of Duties Hold and answer the crisis line on a rotating schedule to provide in-the-moment counselling and crisis support, provide consultation, or determine eligibility for the Peel Crisis Capacity Network. Facilitate referrals, attend community case conferences and provide consultations to other organizations for people deemed ineligible for the Network. Conduct bio‑psychosocial assessments with all clients referred to the Network who meet primary eligibility requirements in a timely manner—over the phone, via virtual means or in person as applicable. Develop, negotiate, and process service contracts amongst clients, family members and service providers in relation to Network supports. Provide consultation and clinical support to clients, families, service providers/Network partners as required to facilitate crisis resolution. Develop and present clinical suggestions to clients, families, and service providers regarding ongoing support needs. Organize, lead and participate in case conferences for clients, families and agencies regarding clients accepted into the Network. When appropriate, allocate short‑term and goal‑oriented PCCN flexible funds for services to be provided to clients to support a clinical plan. Participate in appropriate Lead Agency, Network Partner and Advisory Committee meetings in relation to PCCN service delivery. Active participation in committee meetings to provide insights into the developmental sector, contribute to discussions, and support decision‑making processes. Improve and resolve issues in PCCN Memorandums of Understanding (MOU) and other relevant service agreements based on identified needs from Network Partners and the community, in collaboration with the Coordinator. Deliver community presentations on dual diagnosis, the role of PCCN, and strategies to enhance awareness and support for the developmental sector. Represent PCCN in community engagement opportunities. Maintain ongoing engagement in the Continued Competency Program throughout the year, ensuring compliance with the Ontario College of Social Workers and Social Service Workers' professional development standards. Maintain competing demands in supporting a case load—client crises, service provider requests, data entry and managing scheduling priorities. Facilitate 8‑10 week psychoeducational parent/caregiver support groups. Complete and maintain professional documentation including assessments, case notes, attending meetings, discharge summaries and statistical information. Actively attend monthly team meetings and bi‑weekly clinical meetings. Contribute to the ongoing development of PCCN and the best practices. Demonstrate effective use of technology. Perform other duties as assigned. Qualifications Bachelor of Social Work or Master of Social Work. Registered or waiting acceptance (documentation required) with the Ontario College of Social Workers and Social Service Workers. Counseling experience through employment, an educational placement in developmental disability services, children’s mental health services and/or adult mental health services. Comfortable providing support to families and clients through various service models (phone, videoconferencing, in‑person meetings). Comfortable providing support to families and clients in various service locations such as in the family home, community and other settings as needed. Ability to effectively work as a part of a team and in an independent setting. Strong assessment skills and the ability to frame interventions within a family systems perspective. Comprehensive knowledge of developmental services and community mental health networks. Excellent organizational and interpersonal skills with proven ability to work effectively at a client, program and systems level. Ability to facilitate effective meetings through various formats. Ability to maintain competing demands such as client crisis, service provider requests, data entry and maintaining scheduling priorities. Excellent analytical, interpersonal and communication skills (oral and written). Highly proficient with PC computer environment; MS Office (Word, Excel, Outlook, etc.). Working hours are 9:00am‑5:00pm, however some evening work may be required. Vulnerable Persons Criminal Reference check required. Current valid First Aid/CPR certificate required. Must have a Valid Ontario Driver's License (Class G or G2) and access to a reliable vehicle and insurance. Fluent in French (or additional second languages) an asset.
