Detailed Overview Supporting the Vision, Values, Purpose, and Commitments of Fraser Health, the MHSU Hospital Community Social Worker Liaison provides recovery‑oriented services and ensures linkage between hospitals and community treatment and support services. The role provides crisis counseling, suicide prevention and treatment, outreach, and short‑term follow‑up as required to reduce demand and bed utilization in hospitals and to provide more effective care and treatment for adults in the community. Responsibilities Provide linkage between acute‑care hospitals and community services by participating in discharge planning rounds, case conferences, and follow‑up with patients discharged from the hospital. Meet with mental health case managers to discuss current caseloads and new referrals, and ensure clients released on extended leave are linked with community physicians and mental health case managers. Conduct psychosocial assessments and provide short‑term crisis counseling and follow‑up treatment, including suicide prevention and intervention, to clients and their families. Use a variety of interviewing, counseling, and assessment methods in accordance with professional standards and clinical policies. Provide referrals, in accordance with established protocols, for mental health clients not requiring hospitalization to appropriate community treatment alternatives and services. Act as a resource for patients, families, and other team members. Maintain client records and statistics, prepare reports, and complete forms as required in accordance with regional policies and standards, including clinical record standards of Mental Health Services. Obtain consent forms for release of client information. Provide mental health community case work information and expertise by attending meetings and participating on committees as required. Participate in research and quality improvement activities by identifying patient care issues, collecting data, and recommending corrective action or changes to practices, procedures, or protocols. Participate in and attend in‑services and other educational programs. Identify own learning goals and maintain and update current clinical competence and knowledge within area of practice. Maintain current knowledge in the mental health field through reviewing literature, consulting with experts, and engaging in professional development activities to improve care through new practices. Perform other related duties as assigned. Qualifications Education, Training & Experience Graduation from an accredited Social Work program. Full registration with the BC College of Social Workers. Two (2) years’ recent, related experience in a community or hospital psychiatric setting, or in a rapid‑response crisis stabilization function, or an equivalent combination of education, training, and experience. Valid Class 5 BC Driver’s License and access to a personal vehicle with appropriate insurance coverage. Skills & Abilities Ability to complete initial and ongoing mental health and substance use client assessment through appropriate/prescribed technical, therapeutic, safety‑type interventions including crisis intervention. Knowledge of mental illness and substance use treatment. Counseling and group facilitation skills. Knowledge of legislation and government policies and procedures that relate to mental health and substance use practices. Effective verbal and written communication with clients, families/caretakers, the public, medical staff, and members of the interprofessional care team. Promotion of person‑centered care demonstrating empathy for clients/families/caretakers, sensitivity to diverse cultures and preferences, and advocacy for special populations. Education of clients and families/caretakers about topics essential to their health care and well‑being. Organization and prioritization of work. Collaboration and promotion of cooperation among interprofessional team members. Use of factual information and core principles to support clinical decisions and actions with relevant research‑based evidence. Operation of related equipment, including relevant computer applications. Physical ability to perform the duties of the position.
