Bilingual Care Coordinator, Regular Full-Time, Congregate Care Team East, Labelle (1.0) role at Ontario Health atHome We are looking for a registered nurse, physiotherapist, occupational therapist, social worker, dietitian, or speech-language pathologist who is passionate about supporting patients to thrive in their homes. About the role As a Bilingual Care Coordinator you will assess patient care needs, develop and implement care plans, and coordinate community services. You will work from office, community or health facility locations and collaborate with a large network of regulated health care professionals. Primary Responsibilities Complete comprehensive patient assessments using validated tools. Assess patient and caregiver understanding and learning needs. Assist patients and caregivers in setting appropriate goals. Collaborate with the care team to develop individualized care plans. Coordinate delivery of home and community care services. Coordinate vendor and resource utilization (medical equipment, supplies). Facilitate communication among multidisciplinary team members. Initiate and authorize care plan implementation with contracted providers. Provide regular reports to physicians, service providers, patients and caregivers. Document case information in compliance with Ontario Health atHome policies. Qualifications Registered Nurse (BScN or diploma) and registration with the College of Nurses of Ontario, or Physiotherapist with registration with the College of Physiotherapy of Ontario, or Occupational Therapist with registration with the College of Occupational Therapy, or Social Worker with registration with the Ontario College of Social Workers and Social Service Workers, or Speech‑Language Pathologist with registration with the College of Audiologists and Speech‑Language Pathologists, or Dietitian with registration with the College of Dietitians of Ontario. Minimum two years of related community‑based professional experience. Proof of COVID‑19 vaccination required. Fluency in English and French. Valid driver’s license and reliable vehicle access. Vulnerable Sector Check completed within the last six months. Desired Competencies Knowledge of medical diagnoses, prognoses, and symptom management. Case management principles and best practices. Understanding of resource availability, costs, and budgeting. Knowledge of relevant health care legislation. Understanding of care delivery in congregate settings. Effective communication and relationship‑building with diverse patients, caregivers and stakeholders. Strong listening and facilitation skills. Diplomacy with sensitive and confidential matters. Computer proficiency (email, internet, Microsoft Office). Work Schedule & Compensation Hours of work: Monday‑Friday, 8:30 am‑4:30 pm (35 hrs/week). FTE: 1.0. Starting hourly pay: $44,480–$47,517.
