Provides consultation services to internal and external health care professionals and applies advanced theory to assess, plan, implement and evaluate the delivery of pharmaceutical care. Develops and evaluates clinical, education and research activities within the designated specialty area, including teaching to students and pharmacists, participation in Medication Use Evaluation, treatment policies, continuing education programs, literature reviews and clinical drug research.ResponsibilitiesProvides consultation services and applies advanced theory to assess, plan, implement and evaluate pharmaceutical care in the designated specialty area.Provides clinical direction on complex cases, including evidence-based drug therapy evaluation, attends patient rounds, reviews medication histories, provides drug information and discharge consultations, and makes recommendations per policies.Ensures quality assurance programs and adheres to departmental guidelines; conducts audits and implements corrective actions; participates in quality improvement activities and prepares reports as required.Provides academic teaching to students and residents; coordinates teaching activities with university programs.Identifies and initiates clinical drug research, coordinating research activities and disseminating findings.Conducts pharmacological education/in-service sessions and orientation for staff.Participates in formulary decisions and policy-making through relevant committees and documentation.Plans, implements and evaluates cost-effective therapy programs and policy standards for the designated area.Represents the department on multidisciplinary committees and at external events.Provides direct pharmaceutical care to patients, identifies and resolves drug-related problems, and educates patients and families about medications.Performs other related duties as assigned.QualificationsEducation and ExperienceCurrent registration as a Pharmacist with the College of Pharmacists of British Columbia.
