OverviewLicensed Practical Nurse, Hemodialysis - Hemo Dialysis - ARH role at Fraser Health Authority. Casual position located in Abbotsford, BC. This role requires post-graduate specialty education in Nephrology; if not completed, indicate interest in pursuing Specialty Education. Fraser Health offers sponsorship for RNs/LPNs to gain specialty education through an in-house BC Renal program.Salary range: CAD $32.84 - $44.96 per hour. ResponsibilitiesAssesses, plans, implements, and evaluates patient care during pre-dialysis, intra-dialysis, and post-dialysis periods in accordance with clinical decision support tools and resources. Establishes and maintains a therapeutic relationship with patients and significant others, ensuring patient autonomy in decision-making and care planning.Contributes to development and updates of the individualized plan of care; attends multidisciplinary care planning meetings and family conferences; advocates for patient and family interests. Performs nursing duties in collaboration with the Registered Nurse and nephrology team prior to dialysis.Plans and prioritizes client care, responding to changes in patient needs within scope of practice; performs hemodialysis nursing duties per LPN competencies; escalates care as needed and collaborates with the RN and team.Facilitates patient teaching to increase understanding of the care plan, chronic disease management, and nursing procedures; collaborates with the interdisciplinary team to optimize teaching strategies.Maintains documentation and updates information to ensure client safety; administers medications and provides education in line with policies and procedures.Participates in quality improvement and program activities to maintain consistent care; ensures a safe working environment and adheres to safety, infection control, and emergency procedures.Assists in orientation and education of new staff and students, in collaboration with the Manager or designate; performs other related duties as assigned. QualificationsEducation and ExperienceGrade 12, plus graduation from an approved Practical Nursing program, and successful completion of post-basic education through Fraser Health in-house program affiliated with BC Renal; plus one (1) year of recent related experience as an LPN in acute care or an equivalent combination of education, training and experience. Current full practicing registration with the British Columbia College of Nurses and Midwives (BCCNM) and current CPR Level C or equivalent and First Aid certificates. Ability to communicate effectively, verbally and in writing.Physical ability to perform duties.Ability to work independently and with others.Ability to operate related equipment.Ability to organize and prioritize tasks.Ability to establish and maintain rapport with clients.Ability to observe and recognize changes in clients.
