OverviewThe Transplant/Nephrology/Urology inpatient unit is looking for a Registered Practical Nurse (RPN) to work in a fast-paced inpatient setting. The unit is a medical surgical unit with a patient population from a variety of specialties including Kidney Transplant, Nephrology, and Urology. The RPN works within a changing environment and in inter-professional teams to advocate for and promote wellness while ensuring the delivery of safe, effective and ethical nursing care. The RPN shares knowledge and expertise with others and contributes to the advancement of nursing practice excellence. The RPN continually enhances practice and competencies through ongoing learning, education, experience and reflective practice. Responsibilities Provide holistic, evidence-based, culturally appropriate, patient-centred quality care in accordance with the College of Nurses of Ontario standards and Unity Health Toronto's mission, vision, values and policies. Optimize the patient's experience and care outcomes by being accountable for one's decisions and professional practice. Establish a therapeutic relationship with the patient. Provide autonomous nursing care with patients considered stable and with predictable outcomes. Collaborate with other members of the patient's care team. Coordinate and communicate patient care activities to optimize the patient\'s experience and outcomes. Collaborate and consult with Registered Nurses and other care team members in the care of patients with increasing instability, and, when necessary, safely transfer care to the Registered Nurse. Patient Care The RPN has four key areas of responsibility: Acquire information to assess patient care and learning needs. Engage patients and families to assess involvement in care and integrate into care planning. Develop a plan of care and integrate nursing activities with other inter-professional activities. Engage in critical inquiry and reflection to select evidence-based interventions. Organize nursing care based on patient care needs and priorities. Continuously gather and analyze clinical data to evaluate the effectiveness of the plan of care and adjust as needed. Anticipate rapidly changing situations, recognize transitions from stable to unstable, establish priorities, and take immediate action in collaboration with the Registered Nurse. Provide health education using effective teaching strategies that consider patient/family learning needs, readiness, and motivation to learn. Collaborate with appropriate team members on patient care needs. Process orders and seek verification from prescribers. Document observations, assessments and care provision to facilitate current and accurate records and effective inter-professional communication. Contribute to discharge and transition planning and other planning activities to facilitate care continuity and coordination; advocate for care needs to be met in the most appropriate setting. Collaboration & Coordination Organize patient care activities while working collaboratively with patients, families and the inter-professional team to achieve agreed-upon outcomes and optimize resource utilization for individual patients. Establish and maintain respectful, collaborative, therapeutic and inter-professional relationships within and across hospital units/programs (and externally where appropriate for the patient population). Coordinate and communicate patient care activities for a safe and seamless transition between shifts or settings; ensure appropriate transfer of accountability between care providers. Participate in hospital committees and other groups to represent the RPN perspective and bridge unit/program and patient needs. Leadership & Professional Development Be accountable for actions and decisions and practice in accordance with professional, legal and accreditation standards and evidence-based practice. Align behavior with Unity Health Toronto\'s vision, mission and values and code of conduct; promote nursing as a knowledge-based profession; support the advancement of professional practice plans and initiatives. Engage in self-reflection, performance review and ongoing learning. Participate in mentoring, teaching and/or coaching of students, co-workers and other regulated and unregulated health professionals. Embrace change that leads to improved nursing practice and better patient outcomes. Advocate for a safe and healthy work environment. Research Participates in or supports research projects. Supports and participates in evidence-based practice implementation and quality improvement initiatives. Qualifications Successful completion of a Practical Nursing Diploma Program. Currently registered as a Registered Practical Nurse with the College of Nurses of Ontario (General or Temporary Class). BLS Certification required. Must have worked as an RPN within the last year. Successful completion of Coronary Care 101 or willing to obtain. Successful completion of Physical Assessment or willing to obtain. Successful completion of Nephrology I or willing to obtain. Demonstrated recent experience and interest in working with Diabetic and Renal patients. Inpatient medical and/or surgical ward experience within the last 2 years or placement experience on inpatient medical and/or surgical ward within the last 2 years. Current experience with the patient population (ESRD, Diabetes and its complications) and post-operative surgical care (Urology) is essential. Demonstrated knowledge in the care and management of patients receiving peritoneal dialysis. Proven planning, time management, organization and coordination skills. Evidence of ongoing professional and educational development. Demonstrated ability to problem-solve and work independently and collaboratively in a fast-paced multidisciplinary setting. Excellent interpersonal, communication and organizational skills. Excellent attendance record. Proven ability to act both independently and as an active member of a multidisciplinary team. Must be available to work all shifts including days and nights, weekends and holidays. New RPN graduates are encouraged to apply.
