OverviewJob Code: 20894Status: Full TimeLocation: SMH Kingston OutreachUnion: ONA PCHWage: $39.07 - $56.00Posting Period: December 5, 2025 - December 11, 2025The Registered Nurse (RN) – Case Manager, Seniors Mental Health (SMH) Outreach, will work as part of an inter-professional specialty SMH Outreach team providing comprehensive assessment; consultation; risk management; and support for older adults living in the community with complex cognitive-mental health needs, often with multiple physical co-morbidities. The RN Case Manager will assist with the development of an integrated delivery of service to enhance the quality of life for older adults with complex cognitive-mental health needs and/or behavioral disturbance through partnerships with various community services supporting seniors across the system of care. The RN Case Manager will coordinate and integrate services and resources to assist the older adult with aging at home and promote system integration. As a member of the SMH Outreach team, the RN Case Manager will practice in the role of Case Manager utilizing the knowledge, skills, and abilities of a full scope Registered Nurse in order to provide specialty service to the target population and also to enhance, augment and develop resources in the community. Education & Experience Bachelor’s degree in Nursing, preferred or an equivalent combination of education and experience Current unrestricted registration with the College of Nurses of Ontario Education completed or experience working in the PIECES framework preferred Minimum of 2 years of recent experience in geriatric psychiatry and/or geriatrics Minimum 1 year of experience working in a community nursing setting, working autonomously to support people to live at home. 3 years preferred.
