Siam Surgery has an opportunity for two care coordinators to join our team on a part-time or full-time basis, for a minimum of 22.5 hours per week.The post-holder will provide extra time, capacity and expertise to our practice team to help us to improve the care provided to our patients.Main duties of the jobWorking with GPs and other professionals within the practice to identify and manage a caseload of patients, particularly vulnerable patient groups with long-term conditions that may require extra support.Directing patients to locally available support and services.Supporting the coordination of all key activity, including access to services, advice and information, as well as ensuring that health and care planning is timely, efficient, and patient-centered.Helping patients prepare for, or follow-up after, clinical conversations and hospital discharges.Promoting health education.Using clinical systems on a daily basis, with confidence to run searches and keep care records up-to-date.About usSiam Surgery is a GP practice and is part of Sudbury Primary Care Network. Located within Sudbury Community Health Centre, we provide a high level of care to over 13,000 patients in Sudbury, Suffolk and its surrounding villages.
