Get AI-powered advice on this job and more exclusive features.Salary CAD $31.56 - $33.28 / hourOverviewWe are currently looking to fill a Casual opportunity for a Client Services Assistant to join our Home Health Team located in New Westminster, B.C.LocationNew Westminster, British ColumbiaExperience and BenefitsNew Westminster is known for its waterfront views along the Fraser River, located on the north side of the Fraser River bordering several municipalities in each direction (Burnaby, Coquitlam, Surrey, and Delta), making it the centre of the Greater Vancouver region.Comprehensive, 100% Employer-Paid Benefits: Enjoy peace of mind with full coverage.Generous Vacation Time: eligible employees can earn up to four weeks of vacation to recharge and relax.Benefit Portability: Seamlessly transfer your benefits from another HEABC employer.Immediate Pension Enrollment: Secure your future with a defined municipal pension plan from day one.Maternity Top-Up: Receive an 87% top-up during maternity leave.TransLink Pass Subsidy: Save on commuting costs with a 50% subsidy on TransLink passes.Additional employee discounts and perks available.ResponsibilitiesCoordinates financial, administrative and contract aspects of client services and care management under the direction of the Manager, Clinical Services or assigned clinical staff.Participates with health care provider in care planning with client by obtaining required client information from a variety of sources and documenting client needs/wants.Gathers and documents information from existing health information systems for all new clients for review by assigned health care professional; identifies additional avenues for gathering information and collects relevant and/or missing data as required; reviews files to obtain historical information.Works with the client and/or caregiver to complete the financial portion of the client's financial assessments and temporary rate reduction requests; documents other personal information to complete waiver forms such as sponsorship or residency forms; coordinates contract, financial and administrative aspects of client services and care management under the direction of the Manager, Clinical Services or assigned clinical staff.Supports long term clients to access community or community related services in accordance with the established care plan; ensures the assigned health care professional is kept informed and reviews materials such as policy and procedures and clinical decision-making tools to assist with various care management issues.Researches community resources and identifies information that will assist in the development and implementation of client services and care management plans; completes standard forms and arranges for services with other community agencies and provincial Ministries such as Income Assistance.Identifies available social, economic, recreational and educational services and resources in the community to meet client needs and provides client with the information; receives client feedback, inquiries and complaints and responds as required; forwards more complex issues to the Manager or assigned clinical staff for follow up.Organizes individual and group client service meetings and schedules community appointments for clinical staff, as required.Works collaboratively with care team in the documentation of client care plans; completes data entry of care plans and other client service items; maintains client records by ensuring that client files, applications forms and financial documents are kept up to date; creates and maintains spreadsheets of client information; completes filing and related administrative duties.Performs other related duties as assigned.QualificationsEducation and ExperienceDiploma in a social or health services field of study such as Community Social Service and two (2) years' recent related experience working with seniors or adults with physical and/or cognitive disabilities or an equivalent combination of education, training and experience.Valid BC Driver's License and access to personal vehicle for business related purposes.Skills and AbilitiesAbility to work independently and in cooperation with othersAbility to communicate effectively, both verbally and in writingAbility to organize and prioritizeAbility to observe and recognize changes in clientsAbility to analyze and resolve problemsConflict resolution and crisis intervention skillsAbility to establish and maintain rapport with clientsAbility to operate related equipmentPhysical ability to perform the duties of the positionAbout Fraser HealthFraser Health is the heart of health care for over two million people in Metro Vancouver and the Fraser Valley in British Columbia, Canada, on the traditional, ancestral and unceded lands of the Coast Salish and Nlaka’pamux Nations and is home to 32 First Nations within the Fraser Salish region.
