Salary The salary range for this position is CAD $41.42 - $59.52 / hour Job Summary We are currently hiring for a Relief Part Time Community Health Nurse - Registered Nurse to join our Specialized Community Services Program - Complex Medically Frail team located in Delta, B.C. This position is available until approximately May 2026 or until the return of the incumbent. Benefits Four weeks of vacation to start Comprehensive 100% employer paid benefits Immediate enrollment in a defined municipal pension plan 87% maternity top-up 50% subsidy on Translink passes Detailed Overview In accordance with the British Columbia College of Nurses and Midwives standards of practice and the Mission and Values of Fraser Health, the Community Health Nurse - Registered Nurse works independently in the community setting. Works collaboratively and as a member of an interprofessional team in the management of an assigned client caseload including assessments, coaching, interventions, client care services and follow up to enable clients and families to live confidently and safely at home and/or community. Responsibilities Establish a therapeutic relationship with the client through interpersonal and interviewing techniques in person and/or over the telephone. Screens referrals and provides individualized client assessments, interprofessional care planning and interventions including clinical care to clients with multiple complex chronic conditions. Develops a comprehensive shared patient/client care plan collaboratively with the interprofessional team and primary care provider. Provides direct client care and identifies other care services required in accordance with applicable guidelines, policies and evidence-based best practice. As required based on the local community model, makes decisions on client specific direct care tasks, assigns tasks to Community Health Workers and provides training and support to ensure competency. Facilitates care conferencing to review client care plans and determine referrals to other services. Discusses clinical findings with client/family, develops action plans, sets goals and accesses system information to review client data. Supports clients and families before, during and after interprofessional conferences and clinic visits by providing information and advocating on behalf of the client/family. Arranges and participates in joint home visits to clients and families with other healthcare professionals as required. Provides health counseling to clients through education, self-management, self-monitoring and wellness promotion/prevention. Maintains clinical records such as intake screens, client assessments, clinical interventions and care plans. Maintains professional practice growth, knowledge and expertise by reviewing relevant literature and attending educational workshops. Participates in department quality improvement and risk management activities by identifying client care issues and collecting data. Participates in the orientation and ongoing education of nursing staff and students by providing information as a preceptor when appropriate. Provides input in the development and revision of standards of care, policies and procedures and advocates for improvements in clinical practice. Performs other related duties as assigned. Qualifications Education and Experience Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives. One year recent related clinical experience assessing and treating complex geriatric and/or adult patients with chronic health conditions in an acute or community/outpatient care setting. Valid BC Driver's license and access to a personal vehicle for business-related purposes. Skills and Abilities Demonstrated knowledge, skills and competence in gerontology and adults living with complex frailty and chronic illnesses. Demonstrated ability to complete initial and ongoing client assessments and provide nursing care through therapeutic interventions. Demonstrated ability to communicate effectively, both verbally and in writing. Demonstrated ability to integrate and evaluate data from multiple sources to problem-solve effectively. Knowledge of broad health care services and community resources agencies. Ability to promote client-focused care and sensitivity to diverse cultures and preferences. Ability to independently manage and prioritize clients with diverse healthcare issues. Ability to teach clients and others about essential health care topics. Ability to work effectively in a dynamic environment with changing priorities. Ability to work independently and as a member of an interprofessional team. Ability to operate related equipment and applicable software applications. Physical ability to perform the duties of the position.
