Job Code: 20709 | Status: Temporary Full Time (up to 15 months) | Location: AMH Dual Diagnosis Consultation Outreach | Union: OPSEU 4104 Paramedical | Wage: $44.67 - $57.91 | Posting Period: Posted until Filled The Social Worker is a member of the inter‑professional Dual Diagnosis Consultation Outreach Team serving southeastern Ontario. The team provides assessment, consultation, and short‑term treatment for people over the age of 16 with a dual diagnosis (an intellectual disability and a diagnosed or suspected mental illness including behavioral disorders or persons with Autism Spectrum Disorder who have cognitive deficits). Utilizing a shared care model, the team works in collaboration with community partners and family physicians to reduce the impact of mental illness and improve the quality of life for adults with a dual diagnosis and their care providers. As a member of the team, the Social Worker will utilize discipline specific skills as defined by the Ontario College of Social Workers and Social Services Workers (OCSWSSW). The Social Worker will provide assessment, diagnosis, treatment and evaluation of individual interpersonal problems through the use of social work knowledge, skills, interventions and strategies, to assist individuals, couples and families to achieve optimum psychosocial and social functioning. The role of this Social Worker position is specific to providing assessment, counseling, and intervention to individuals living with a tri‑diagnosis (addictions, mental health, and developmental disability) from multiple approaches including the harm reduction approach, recovery‑oriented approach, and trauma‑informed practice. Skills and Qualifications Required Master’s Degree in Social Work, required. Current registration with the Ontario College of Social Workers and Social Service Workers, required. Annual Completion of OCSWSSW Continuing Competence Program to provide evidence of continuing competence to practice social work, according to the guidelines set by the College. 2 years’ experience developing treatment plans and providing therapy to individuals who have a concurrent disorder (substance use disorder and mental illness), preferred. 2 years’ experience working directly with individuals who have a dual diagnosis, preferred. Experience working in a wide range of community settings including clients’ homes. Proven experience in working with families and community agencies. Experience developing and facilitating community development initiatives. Position Responsibilities Provide harm reduction based, trauma‑informed assessment, consultation, intervention and counseling. Facilitate psychoeducational groups and/or individual therapy targeted at decreasing client’s risk of harm in the community. Meeting the client’s where they are at and providing appropriate intervention to reduce their risk of harm in the community. Review and evaluate the treatment and ongoing care plan in consultation with the interdisciplinary team. Review treatment plans with the individual, family members and caregivers / care provider, and identify additional support and education needs. Conduct suicide risk assessments and intervene as appropriate when risk is identified. Identify team and personal education and development needs and participate in the meeting of those needs. Provide short term individual psychotherapy and facilitate groups adapting therapeutic modalities (CBT, DBT, IPT) to accommodate persons with a dual diagnosis.
