Social Worker - Children's Disability Team Our Children's Disability Team works with disabled children and their families, assessing their need for social care services and supporting Child in Need and Child Protection plans for children under Care Proceedings, Children We Care For and Leaving Care adults. This role offers the opportunity to maximize participation and reflect children's rights in service delivery by acting on the views of children and young people. Responsibilities Assess need and provide focused work to help parents and children make changes, recording and evaluating outcomes and liaising with other agencies and voluntary organisations. Serve as the allocated worker for disabled children identified by assessment and for disabled children who are looked after. Provide ongoing timely assessment, planning, implementation and review for allocated cases, working in partnership with young people, families and partner agencies. Prepare written reports for review, case conferences, courts and departmental panels as appropriate. Undertake effective analysis and management of risk for allocated children and promote and support their developing needs under the supervision of the Team Manager. Qualifications A Qualified Social Worker, registered with Social Work England. Kind, caring, with an understanding of and respect for individuals qualities, abilities and diverse backgrounds. Advocating for equality and integrity in all actions. Always learning, passionate to grow and develop skills.
