Join an innovative and unique primary health care team to provide comprehensive, integrated, and trauma‑informed care. Our model of care offers holistic team‑based care by supporting physicians, nurse practitioners, nurses, social workers, clinical counsellors, allied health professionals and administrative support. Together this team will extend continuous care that will be intentional and coordinated to all members of the community, including the vulnerable and medically complex. UPCCs are dedicated to delivering timely, person‑centred, culturally safe, trauma‑informed longitudinal and urgent primary care. The team includes physicians, nurse practitioners, nurses, social workers, clinical counsellors, allied health professionals and administrative support. We offer a clinical enhancement program with regular in‑house skills development such as 12‑lead ECG interpretation, point‑of‑care ultrasound (POCUS), suturing and x‑ray interpretation, plus centralized NP orientation. Responsibilities Diagnoses and treats patients for undifferentiated diseases, disorders, and conditions within scope; writes orders and provides first‑line care in emergencies. Monitors ongoing care, orders appropriate screening and diagnostic investigations, interprets reports, and plans treatment. Establishes priorities for management, provides follow‑up treatment, and communicates with patients and families about findings, diagnoses, outcomes, and prognoses. Collaborates and consults with physicians or other providers to assess and diagnose patient status; implements treatment plans; may admit and discharge patients per policy. Prescribes drugs within statutory limits and employer policies. Assigns work, evaluates performance, and provides education and supervision as necessary; may hire staff and evaluate performance. Participates in research contributing to improved patient care and advances in nursing and population health. Maintains population health focus by implementing screening and health promotion activities for at‑risk populations. Participates in interdisciplinary staff and nursing education through case presentations, mentoring, role modeling, and knowledge exchange. Develops, implements, and evaluates policies and procedures related to nursing and health system practices. Performs other duties as required. Qualifications Education and Experience Current practicing registration as a Nurse Practitioner with the British Columbia College of Nurses and Midwives (BCCNM). Recent relevant clinical nursing experience. Assess and recognize population health trends; plan and implement strategies for population‑based prevention and promotion. Implement and evaluate planned change. Supervise and evaluate the care delivered by others. Lead and work within a team. Self‑direct, interact and adapt effectively with other professionals in complex, dynamic situations. Transfer knowledge, teach, coach and mentor others. Identify and respond appropriately to legal and ethical issues in patient care. Self‑assess performance and assume responsibility for professional development and consultative assistance.
