The Manager of Allied Health supports the Community Bundled Care and Integrated Care Solutions (ICS) programs by providing clinical leadership, professional practice oversight, and operational support. The Manager also plays a key role in supporting safe transitions from hospital to home, collaborating with hospital partners, internal teams, and community providers to ensure timely, coordinated, person and family centered care. What You’ll Do Provide clinical leadership and act as a resource for ICS and bundled care teams. Support safe transitions from hospital to home, including assessments, care plans, and follow ups. Lead or support clinical projects, quality initiatives, and digital health adoption. Conduct community visits for complex cases and resolve client or family concerns. Oversee onboarding, coaching, and performance reviews for field staff. Collaborate with intake, coordination, and scheduling teams to ensure timely care delivery. Manage incident reporting, safety screening, and compliance with regulatory standards. Attend hospital huddles, community meetings, and case conferences. Participate in rotational clinical on call and complete additional tasks as needed. Why Join Bayshore?
