Job Code: 20967 Status: Temporary Full Time (up to 18 months) Location: Providence Care Hospital Sites Union: ONA PCH Wage: $42.51 - $61.11/hr Posting Period: Open until filled Reporting to the Patient Flow Director, the Patient Flow Coordinator plays a key role between Providence Care's staff, physicians and external partners in ensuring seamless, quality and efficient patient flow activities. The Coordinator responds to inquiries from key stakeholders directly and/or indirectly and assists the Manager by researching and providing requested data or background information, as requested. The Coordinator problem solves barriers to transitions in care by working closely with the Access to Care and Transition and the clinical program teams to identify solutions. The Coordinator completes daily, weekly and monthly summaries provided to relevant leaders in the hospital and is fully informed of issues and challenges that require action. Utilizing sound clinical judgment and strong assessment skills, the Access and Flow Specialist screens and evaluates potential candidates for admission, collaborating with patients, families, care teams, and community organizations to coordinate person-centered transitions. The role requires adaptability and responsiveness to evolving system pressures and patient needs, often supporting flow initiatives as they evolve. Skills And Qualifications Required Baccalaureate degree in Nursing, preferred or an equivalent combination of education and experience Master's degree in health studies from a recognized educational institution, preferred Current, unrestricted registration with the College of Nurses of Ontario Two years of experience in providing clinical practice in a hospital setting with the relevant population.
