About UsCanadian Mental Health Association Durham is an integrated community mental health and primary care hub that has been providing mental health services to Durham Region for over 60 years.During that time it has evolved to provide a broad range of services to individuals and their families experiencing mental health challenges from information and education to case management, psychosocial rehabilitation, housing and primary healthcare. These services are provided through inter‑professional teams focused on providing clinically based services that, in addition to dealing with client mental health issues, address social determinants of health using the recovery model.Purpose of the PositionReporting to the Performance Manager HART Hub & French Language Services (FLS), this Lead will oversee the day‑to‑day operations and management of HART Hub. The role includes collaboration with the Mission United Program and sustaining effective partnerships for optimal service planning at any point in a client’s HART Hub pathway.LocationHART Hub employees may begin work at 60 Bond St. West and will move to the designated HART Hub location in Oshawa at a later date.Duties and ResponsibilitiesProgram ManagementEnsures that the day‑to‑day services are planned, delivered and evaluated by the multidisciplinary team in accordance with professional standards, good operating principles, best practices and legislative requirements.Directs and coordinates client onboarding, treatment and discharge processes.Participates in cross‑functional teams developing, recommending and implementing short‑ and long‑term plans for the program and organization.Collaborates with HART Hub program and administrative leads to ensure consistency with policy and guideline adaptations as the program continues to evolve.Continually evaluates the status of clients and appropriately plans and coordinates treatment activities with input from the multidisciplinary team to ensure attention is given to their changing needs.Prepares, maintains and monitors monthly and annual statistics as required by the funder.Oversees and audits the accuracy of maintenance of HART Hub documented client information as per CMHA Durham policy and procedures.Oversees and manages departmental budgets and expenses to ensure appropriate funding allocation.Performs other tasks as assigned by management.People ManagementOrganizes the day‑to‑day activities within HART Hub in collaboration with medical services, managing a high‑performance multi‑service team of employees and preparing work plans that align with the organization’s goals and objectives.Directs employees by providing support and guidance to facilitate medication management, psychiatric rehabilitation, activities of daily living, goal planning, clinical assessments, community resources, client intakes, discharges and other relevant program‑based tasks as they arise.Responsible for adhering to documented employee management processes including recruitment, establishing goals, strengthening competencies and supervising/evaluating performance.Accountable for work schedule management, approval of timesheets, absence coverage (vacation, wellness time).Creates and contributes to a positive work environment where all employees can maximize their job satisfaction and productivity.Oversees employee coverage to meet client care needs.Maintains positive relationships with community partners and promotes/facilitates community service integration as required.Facilitates meetings focusing on business and operational issues.Client SafetyAdheres to CMHA Durham client safety programs including program policies, emergency procedures, infection control and prevention, and partnership with clients, family members and fellow employees. Maintains open lines of communication to assist the organization in identifying and achieving quality outcomes for client safety. Reports client safety incidents and suggested improvements to manager or delegate.Other Duties as AssignedAttend and participate in team meetings and all employee meetings.Participate in organizational quality improvement activities, e.g. accreditation projects, committee work, special events.Participate in quality improvement activities at the team/program level.Participate in external committees as required.EffortSits at desk, stands and walks daily to perform the duties of the job.Requires concentration, conceptualization and attentiveness daily to answer all inquiries.Operates personal vehicle to perform the duties of the job.Requires creativity and innovation daily using sight, hearing and touch to operate a computer.Develops strategic actions and provides information on policies and procedures.Proofreads, performs comprehensive assessments and reads documents.Resolves conflicts, problem solves, facilitates meetings, attends meetings, oversees daily operations, manages caseloads, prepares documents, listens to clients, etc.Risk ManagementMinimizes risks by adhering to CMHA Durham Policies and Procedures, identifying, assessing, evaluating and controlling risks to ensure the well‑being of employees, clients, partners, staff and visitors.QualificationsPost‑secondary education and a minimum of 5 years clinical experience in mental health and a minimum of 5 years related management experience, and/or an equivalent combination of education, experience, and/or lived/life experiences.Sound knowledge of the Ontario Mental Health Act and the Landlord Tenant Board Act.Confidence when communicating with community service providers and professionals (e.g. probation officers, lawyers, landlords, police).Good knowledge of psychiatric assessments in mental health, especially in relation to current and future clinical programs and services.Extensive knowledge of client information management in mental health services.Proven ability to provide employee supervision and team leadership.Experience with methods of program evaluation.Ability to maintain confidentiality.Ability to work with individuals with diverse backgrounds.Ability to work independently and co‑operatively as part of a team, management, proficient in crisis management, intervention and recognition of unsafe environments.Expert level administrative and organizational skills.Excellent written and verbal communication skills to ensure staff understand and contribute to the strategic vision of the organization.Proficiency in MS Office documentation software and the internet.Able to integrate CMHA Durham’s philosophy of being a welcoming organization into everyday practice.Ability to adapt to change and navigate through ambiguous situations.Knowledge of Indigenous communities and cultural practices (preferred).Intermediate fluency in French considered an asset.Work outside of normal business hours as required in compliance with the Disconnect from Work policy.Working ConditionsWorks on site in office settings and off site in the community, public environments and residential settings.Works in a low‑barrier community treatment environment.Exposure to clients in crisis.Exposure to hazards such as blood/bodily fluids and cleaning fluids.Some exposure to hazards such as needles, medications, etc.Exposure to unpredictable psychiatric behaviors that may be considered offensive (abusive/irate clients), deadlines, multiple demands, distractions/interruptions.What we offerCMHA Durham is a non‑profit community‑based organization that believes that mental and physical health care is for everyone regardless of class, race, sexuality, gender, age, faith or ability.
