This is currently a vacant position to support a growing program.This role represents an exciting opportunity to be part of a growing program that aims to have a major impact on the caregiver experience in Ontario. You will be helping to enhance existing social prescribing principles to be more inclusive of the caregiver experience. The ideal candidate can work independently with minimal supervision, as well as on a team, and be able to carry out both program management and direct service delivery duties within the community. This is a remote first role that will require occasional travel to support in person presentations and deliver caregiver support within clinic or community settings.About Social Prescribing:Social Prescribing is a holistic approach to healthcare, bringing together the social and medical models of health and wellness and providing a formal pathway to support. You can find more information on these principles through the Canadian Institute for Social Prescribing: https://www.socialprescribing.ca/Key ResponsibilitiesReporting to the Manager, Caregiver Navigation and Workplace Programs, the Caregiver Link worker will maintain existing partner relations and establish new community-based social prescribing partnerships focused on supporting caregivers within integrated primary care and other community health settings.You will be helping to expand on new models of social prescribing for caregivers in Eastern Ontario.Provider EngagementIdentify and develop key partnerships with integrated primary care teams, community paramedicine or other community health servicesProvide live in-person information sessions and lead in-person and virtual meetings with community partnersConnect with health and community service providers and work alongside them to identify and support caregiversVirtually connect with Francophone providers in other regions outside of Eastern ON as needed.Project ManagementIdentify potential partner sites and build caregiver social prescribing practices within each site with a specialization in supporting Francophone caregiversImplement strategies, processes, and practices alongside partners for service delivery and referralsIndependently manage project deadlines, workflow, and daily service delivery requirementsCollect required data points and develop reportsProvide input to strategic planning for the program and organization as a wholeDirect to Caregiver SupportManage incoming and outgoing referrals and communications in a timely manner following OCO service policies and protocolsProvide direct support to caregivers in-person, by phone, and email in French and English, listening to their needs and co-developing intervention optionsProvide resources or program options for their care recipient as neededAssist caregivers in navigating OCO programs as well as support within their communityComplete service readiness screening and program registrations as requiredQualificationsMinimum 5 years’ experience in primary care, community health settings, or clinical environments within community, health care, non-profit, and/or social service settingsExceptional bilingual communication skills – verbally, in writing and in virtual environmentsHighly skilled in supporting community members with complex needsExcel at facilitation of workshops, focus groups, network meetings and outreachProject or program management experience, managing deadlines and reporting requirementsAbility to develop and maintain partnerships with external groups and providersAssessment capabilities to determine best service matchAbility to navigate community and publicly funded supports and servicesStrong conflict resolution skills, ability to manage challenging situationsComfortable working with vulnerable community members facing hardshipExperience with following privacy and consent guidelinesSelf-starter, solution focused with demonstrated problem-solving skillsKnowledge of caregiver needs, the role of caregivers in the health and community care systems, and/or experience as a caregiver is an assetStrong technical ability and proficiency in the use of computers and various software applications, in particular Microsoft Office, Dynamics 365, Survey Monkey, Eventbrite, Zoom, Ocean, CaredoveAbility to speak and write in French is an assetAbility for occasional work after hours or on weekends to accommodate pre-arranged meetings or eventsAbility to travel within Ontario, Valid G license and access to an insured and reliable vehicle,EducationBachelor’s degree in nursing, social work, social services or similar disciplineRegistered Social Worker, Registered Social Service Worker, Registered Nurse or Registered Practical Nurse in good standing is an assetThe posting will close at 12:00 noon on February 11, 2026.
