You will be helping to enhance existing social prescribing principles to be more inclusive of the caregiver experience. The ideal candidate can work independently with minimal supervision, as well as on a team, and be able to carry out both program management and direct service delivery duties within the community. This is a remote first role that will require occasional travel to support in person presentations and deliver caregiver support within clinic or community settings.About Social Prescribing:Social Prescribing is a holistic approach to healthcare, bringing together the social and medical models of health and wellness and providing a formal pathway to support.
