Clinical Pharmacist - Abbotsford Regional Hospital and Cancer CentreThe salary range for this position is CAD $51.37 - $64.19 / hour.We are seeking a Clinical Pharmacist to join our team at Abbotsford Regional Hospital and Mission Memorial Hospital located in Abbotsford and Mission, BC.We are a collaboration of four premier health employers: Fraser Health, Provincial Health Services, Providence, and Vancouver Coastal health. We provide an integrated approach to pharmaceutical care and service excellence for our clients.As a Clinical Pharmacist, you will work to provide the following services to our patients:Individualized pharmacotherapyCounselling patients about their medication therapyWorking in collaboration with physicians and other health care providers to develop, implement, and monitor therapeutic plansAssisting in the development of drug therapy guidelines for individual disease conditionsPreparing and submitting for publication in professional journals, case studies, and articles that share new knowledgeActing as a department resource person within your assigned areaParticipating in medication use evaluations, quality improvement initiatives, and clinical drug trialsProviding mentorship and supervision to pharmacy students, residents, and new staffTo join us, you will need:Current registration as a Pharmacist with the College of Pharmacists of British ColumbiaCompletion of an accredited Hospital Pharmacy Residency Program or an equivalent combination of education, training, and experienceWe offer a competitive salary package, including comprehensive health benefits coverage, and a career that offers the opportunity to work in a fast-growing and exciting organization with professionals who are eminent in their respective fields.Our pharmacy team aspires to improve and achieve the best possible care for our patients and improve their quality of life.The Clinical Pharmacist provides clinical pharmacy services, including individualization of patient medication therapy, patient medication counselling, and consultation with physicians, nurses, and other health care providers to develop and discuss therapeutic plans.Responsibilities include:Consulting with physicians, nurses, and other health care providers to develop and discuss therapeutic plansIndividualizing patient medication therapyProviding patient medication counsellingAssisting in the development of drug therapy guidelines for individual disease conditionsParticipating in ward rounds, program team meetings, and provision of pharmacy services at the unit levelActing as a department resource person in the assigned area of responsibilityContribute to quality improvement activitiesPerforming drug distribution duties, including interpreting medication orders/prescriptions and dispensing medicationsQualifications include:Current registration as a Pharmacist with the College of Pharmacists of British ColumbiaCompletion of an accredited Hospital Pharmacy Residency Program or an equivalent combination of education, training, and experienceBroad knowledge of organization policies, procedures, and standards of careKnowledge of evidence-based clinical pharmacy practiceAbility to apply clinical pharmacy knowledge and make decisions with a minimum of supervisionAbility to communicate effectively both verbally and in writingAbout Fraser Health: We are committed to planetary health and value diversity in the workforce.
