Job Title: Physiotherapist

About the Opportunity

The transitional health model supports patients and families who require additional time, services, and support to transition back to their previous living situation by providing an alternate space with intentional programming, building design, and resource allocation.

Key Responsibilities
  • Assesses, diagnoses, and treats patients in a tertiary care setting who present with actual or potential impairment, pain, functional limitations, or disabilities.
  • Encourages, motivates, and assists patients with hygiene and daily living activities, such as bathing, dressing, oral care, skin care, meals, and mobilizing, according to the patient's care plan.
  • Plans intervention strategies by promoting active involvement of the patient, family, or caregiver in establishing physiotherapy goals which are based on assessment findings, prognosis, and evidence-based practice.
  • Participates in interdisciplinary rounds, team meetings, and family meetings regarding patient care, prognosis, and discharge, liaises with medical staff concerning patients' progress and needs.
Requirements
  • A master's degree in Physiotherapy.
  • Current registration with the provincial College of Physiotherapists.
  • Minimum of two years clinical experience.
  • Knowledge of the principles, methods, techniques, materials, equipment, and procedures used in physiotherapy and their application in specific cases.
