Position Summary As part of the Service Delivery Centre (SDC) leadership team reporting to the Regional Director, the Clinical Practice Coach (RN) will provide clinical leadership and expertise for registered nurses (RNs) and registered practical nurses (RPNs) using a coaching, mentoring, and quality improvement approach. Key responsibilities include coaching visits, education and competency assessment, orientation and onboarding of new staff, chart audits, and quality improvement. This role will promote, sustain and maintain excellence in client care and professional practice, supporting person‑ and family‑centred, safe care. This position may involve an on‑call component of after‑hours, weekends, and holiday accessibility, with coverage on a rotational basis. Why join SE Health? Responsibilities and Accountabilities Lead clinical orientation and preceptorship of new staff. Review Nursing Practice Questionnaires (NPQ) and develop personalized learning plans. Educate, assess, develop, and maintain clinical skills and competencies for individual nurses, ensuring appropriate skill mix for service delivery. Maintain accuracy of employee clinical competency profiles. Implement and track annual education plans, reporting completion rates. Plan and execute annual clinical skill day events for re‑evaluation of nurse performance. Participate in knowledge transfer activities supporting the use of clinical knowledge tools and evidence‑informed practices. Conduct coaching visits with staff in home or clinic to influence and improve quality of care. Provide 1:1 and/or group clinical coaching and feedback. Conduct chart audits, identify, plan, implement and evaluate improvement initiatives. Use audit results and other evidence to identify gaps and develop continuous quality improvement plans. Collaborate with RD, SDC leadership, CPRT, and APCs to improve clinical quality outcomes and define improvement activities. Participate in Saint Elizabeth’s broader Clinical Support Network supporting organizational quality and clinical excellence. Requirements Registered Nurse (RN) designation. Bachelor of Science in Nursing. Good standing with the College of Nurses of Ontario. Strong generalist clinical nursing background. Minimum one year community nursing experience. Preferable: 2+ years adult education experience, RNAO membership, additional education/training in adult learning or coaching, clinical specialty certifications. Demonstrated commitment to ongoing professional development and alignment with Saint Elizabeth’s values and core competencies. Excellent interpersonal and written communication skills. Outcome‑oriented with ability to achieve desired results. Ability to thrive in a fast‑paced environment with competing priorities. Advocate for and effectively manage change. Initiative, independent and collaborative, building positive relationships. Comprehensive knowledge of the organization’s programs and services. Strong computer skills (PowerPoint, Excel, MS Word, etc.) and familiarity with mobile technologies. Flexibility in work hours and assignments, days and evenings as needed. Valid driver’s license, vehicle, ability to travel within the North York & Toronto area. Additional duties and responsibilities as assigned; covers the Central Toronto region.
