Job Summary The Skin and Wound Clinical Lead is a specialized nursing professional responsible for delivering expert skin and wound care for patients at the Kingston Transitional Care Unit. The Skin and Wound Clinical plays a critical role in promoting skin integrity, preventing pressure injuries, and managing complex wounds through evidence-based practices. Working collaboratively with interdisciplinary teams, the Skin and Wound Clinical conducts comprehensive skin assessments, completes all Hospital-Acquired Pressure Injury (HAPI) audits for all patients, develops individualized care plans, conducts regular chart audits, completes incident reporting (RL-6), completes Ankle Brachial Pressure Index (ABPI) assessments required and provides direct wound care interventions. This role also involves educating patients, families, and staff on best practices in skin health, wound prevention, and treatment protocols. Duties And Responsibilities Perform comprehensive skin and wound assessments for patients in community and/or transitional care unit settings. Conduct 100% of HAPI audits, and document findings. Develop and implement individualized care plans to maintain skin integrity and promote healing. Deliver direct wound care interventions using evidence-based practices and advanced treatment modalities. Monitor patients for early signs of pressure injuries and other skin-related complications. Implement preventive strategies, including repositioning schedules, pressure-relieving devices, and nutritional support. Utilize a wide range of clinical skills, including but not limited to wound care, ostomy and continence care, intravenous therapy, palliative, and health teaching towards chronic disease prevention. Support with wound care supply needs. Provide education to the staff on correct wound care supplies and validation of wound care competencies. Demonstrate autonomy and flexibility to provide a holistic approach to care. Participate in increasing your skills through our comprehensive training, mentorship and preceptorship programs. Teach and delegate tasks and functions to various levels of staff and client family members while maintaining our industry’s highest level of practice. Maintain accurate and timely documentation of assessments, interventions, and outcomes in patient records, including HAPI audits. Ensure compliance with regulatory standards, infection control protocols, and organizational policies. Participate in audits and quality improvement initiatives related to skin and wound care. Monitor and evaluate outcomes of wound care interventions to identify areas for improvement and monitor wound care dashboard. Contribute to evidence-based practice by staying current with research and integrating new techniques. Assist in developing protocols and guidelines for skin wellness programs. Provide direct practice to patients requiring wound, ostomy and continence assessment, care and management. Participate in ongoing internal and/or external continuing education activities. Adhere to Bayshore Policies and Procedures. Participate in quality activities and continuous improvement initiatives in keeping with the company's Quality Management System. Participate in proactive Health & Safety activities while performing all duties. Is responsible to notify immediate Supervisor of any Health & Safety risks or concerns. Maintain confidentiality of client and corporate information and discuss the same only with appropriate Bayshore personnel. Complete other tasks as requested. Education Completed CNA-accredited Skin Wellness Associate Nurse (SWAN) course International Interprofessional Wound Care Course (IIWCC) certification an asset Registered Practical Nurse who graduated from an accredited School of Nursing Registered as a member in good standing with the College of Nurses of Ontario Current Basic Life Support (BLS) required Current First Aid certificate an asset Experience Two years of previous experience in the development of wound care management programs in a hospital or community setting This is a TFT Position for 6 months Primary Location ON-Kingston #J-18808-Ljbffr
