The Physiotherapist plans, organizes and directs diverse programs of treatment, counseling, screening and education for ONTARIO HEALTH ATHOME Champlain Patients in a variety of settings (in homes and retirement homes)Primary ResponsibilitiesProvides physiotherapy services to ONTARIO HEALTH ATHOME Champlain patients through initial assessment, identification of problems, establishment of goals and treatment plans, implementation of physio treatment, follow-up and management of patient’s condition. Establishes priorities and treatment goals with respect to identified problems and develops methods of achieving these goals.

Ensures that clinical records are maintained in accordance with ONTARIO HEALTH ATHOME Champlain Policies and Procedures and the College of Physiotherapists of Ontario. Schedules patient visits according to Physiotherapy priorities. Functions as a member of a multi-disciplinary team and maintains liaison with other health care professionals. Acts as an education and resource person for other health care professionals.

Participates in committees as approved. Partakes in professional development activities and maintains knowledge and ability in the realm of physiotherapy. Maintains supplies and recommends equipment for department use within budget limitations. Interprets the role and objectives of the ONTARIO HEALTH ATHOME Champlain Physiotherapy Program in the community.
