Position Description Reporting to the Director of Health, the KCA Home Care Nurse will provide Home Care Nursing services in the KCA member communities. The Home Care Nurse will work in partnership with other members of the health care team to provide holistic care to the entire community population. The Home Care Nurse will provide assessments, screening, healthy lifestyle support, education and chronic disease management, home care, immunization and communicable disease programming with an ultimate goal of improving health outcomes and facilitating access to services. Position Title: Home Care Nurse Department: Primary Care Initiative Term: Full time Permanent Location: 240 Veterans Drive, 3rd Floor, Kenora, Ontario Reports to: Director of Health Duties and Responsibilities Liaison with other Health Care Providers Consult with clients, Primary Care physician/visiting Physicians and Nurse Practitioners on an ongoing basis; Work closely with other Health Care Providers in the community; Provide referrals to various community programs and supports; Provide referrals to outside agencies and supports as needed; Act as a patient advocate when needed. Health Assessments Perform a comprehensive and focused health assessment that includes health history, health status, risks and considers the psychosocial, emotional, cultural and spiritual dimensions of the client; Understand social determinants of health; Develop care plans; Assess the needs for advanced care and medical directives; Complete and document a thorough medication assessment; Review laboratory and diagnostic reports for clients and flag potential concerns for visiting Physicians and Nurse Practitioners; Health Management and Therapeutic Interventions Provide care for clients with a wide range of health issues; Initiate and/or contribute to development of a care plan; Encourage independence and self-care; Assist and support clients with planning, testing, transportation and in-home care; Provide home care under the First Nation and Innuit Home and Community Care Program guidelines; Initiate consultations with other services and health care providers; Provide follow up care and consultation for clients; Coordinate services and care with clients; Provide interventions as per medical directives or medical orders, such as wound care, staple/suture removal, injections, ear syringing, assisting with procedures, specimen collection (subject to provincial College of Nurses’ standards of practice) and education; Help resolve client concerns. Health Education Assess health education requirements and readiness of the client, including current knowledge, literacy levels, social supports, learning preferences and other factors that may affect education; Acquire, develop and evaluate teaching materials and tools; Provide education related to healthy living, injury prevention, illness, communicable disease, chronic disease, care and treatments; Identify health trends and research that may affect clients. Health Promotion and Prevention of Illness Screening and monitoring to prevent the onset of health issues and/or minimize complications; Provide healthy lifestyle education and support; Provide health education regarding health promotion and prevention. Program Evaluation and Reporting Collect and compile data for reporting requirements, explore avenues to streamline; reporting and ensure effective collection of necessary health indicators in alignment with the KCA member communities, KCA and both federal and provincial governments; Enter information in the Community Based Reporting Template, E-SDRT and E-HRTT and any other reporting requirements for programs; Participate in problem solving issues related to program design, delivery, evaluation and client related issues, monitor performance of the program through appropriate audits and review of operational statistics, audits of policies and procedures and medical directives; In consultation with KCA member communities’ leadership, conduct needs evaluation, program planning, development of the performance and accountability indicators, and compile and report on results; Based on observations, assessments, and monitoring of trends and news, conduct needs assessment and organize training to meet staff and program needs and to continually update and revise orientation and training materials; Write proposals to access resources (funding or materials) for projects; Have a KCA performance appraisal done as per KCA policy; Participate in the Accreditation process, both at KCA and at the community level; Provide proper documentation as per the College of Nurses Documentation Practice guidelines. Program Transformation and Planning Engage with individual KCA member communities’ leadership and health representatives to conduct needs/assets assessments; Assist KCA member communities in proposals for equitable services through funding and restructuring to alternate service delivery models; Negotiate and assist individual First Nations communities with advocating for agreements with federal, provincial, and regional service delivery partners to enhance Community Health Nurse and Primary Care Nurse programming. Position Requirements Certification as a Registered Nurse or Registered Practical Nurse in the province of Ontario with good standing and a member of RNAO with LAP; Understanding and ability to apply Clinical Practice Guidelines as well as other acts and legislation that govern practice; Experience in program planning and evaluation and performance management; Knowledge of Anishinaabe culture and the social determinants of health specific to First Nations populations; Experience and understanding of Indigenous cultural issues, the geographical realities and social conditions within northern remote Indigenous communities; Strong computer skills; Strong written and verbal communication skills; Knowledge and experience in Primary Health Care and social determinants of health; Effective organizational and critical thinking/problem solving skills; Must maintain ongoing professional education; Current CPR/AED with HCP and First aid certification; Understanding of and ability to maintain client and business confidentiality and familiarity with PHIPA; Demonstrated ability to work independently and as part of a team; Must possess cultural and political awareness and sensitivity; Demonstrated sound work ethics; Ability to maintain regular and predictable attendance; Highly motivated, energetic and ambitious; Must be able to work irregular hours, including evenings and weekends; Must be able to work independently with little supervision; Valid driver’s license and vehicle; Current satisfactory criminal reference check with vulnerable sector search; Understanding the language of the Anishinaabe language (Ojibway) is an asset.
