We are inviting applications for the Licensed Practical Nurse position within our Shannex Transitional Health Services Limited team at West Bedford Transitional Health in West Bedford, Nova Scotia. Job Description With the opening of a new building in March 2026, Shannex is expanding its services. The LPN will work collaboratively with interprofessional teams, providing evidence‑based clinical expertise and supporting patients in achieving safe transitions back to their homes or communities. The role involves assessments, medication administration, care planning, documentation, and leadership within the unit. Responsibilities Perform initial and regular health assessments to maintain patient quality of life. Assist with assessing, planning, implementing, and evaluating nursing care for all patients. Ensure compliance with Philosophy of Service and patient care plans to promote optimal health and wellness. Administer and record prescribed medications in accordance with facility policy. Provide consultation regarding patient issues and standards of care to the interdisciplinary team. Ensure Continuing Care Assistants/PSWs complete care tasks and document accurately. Assist patients with hygiene and daily living activities such as bathing, dressing, oral care, skin care, meals, positioning, and transfers. Assist with special patient needs (e.g., eye care, colostomy, catheter). Apply dressings and treatments per physician orders and facility policy. Assist with the development, implementation, and evaluation of care plans. Document in required records (Daily Patient Care Record, Medication Administration Record, progress notes, incident reports). Provide leadership and direction to the team, promoting best practices. Supervise and evaluate the performance of Continuing Care Assistants. Orient and train new team members on departmental standards, procedures, and policies. Coordinate patient care by adjusting assignments according to patient needs and staff availability. Perform additional duties as required. Qualifications Graduate from an approved Practical Nursing Program. Completed Pharmacology and Physical Assessment course (if not already done during diploma). Current registration with provincial Practical Nursing regulatory body. Minimum two years of nursing experience. Demonstrated supervisory skills and ability to evaluate employee performance. Proof of valid CPR & First Aid Certification (required). Mandatory criminal record and vulnerable sector check upon hire. Adherence to Standards of Nursing Practice, Code of Ethics, and provincial regulations. Maintains current knowledge of literature and applied research for health care delivery. Knowledge of current legislation and program requirements. Specialized courses such as Foot Care, Palliative Care, Venipuncture are considered assets. Additional courses like Dementia: Understanding the Journey and Back in Form Lift & Transfer Program considered assets. Bilingualism in French/English considered an asset. Previous experience supporting admission and discharge of vulnerable populations with complex social situations, implementing care plans, and identifying resources.
