The Transitions Home Program (THP) works at achieving timely access to care while enhancing clients’ functional outcomes following discharge from PCH and PTCC. The Transition Navigator will deliver safe, effective, client centered care to clients in their home. The Transition Navigator, reporting to the Program Manager, acts as a fundamental resource to program staff, clients, caregivers and community partners. In collaboration with the program staff, the Transition Navigator triages referrals, assesses clients, supports transition home, provides health teaching and consultation, monitors care plan delivery, connects clients with appropriate supports, and follows up with the client/caregiver post discharge from the THP. The Transition Navigator is accountable for maintaining the Standards of Practice and Ethics of the College of Nurses of Ontario. The Transition Navigator will provide clinical education and experience for student nurses and will participate in formal teaching and research in conjunction with Queen’s University. As a member of the clinical team, the Transition Navigator participates in discipline‑specific and inter‑professional activities that are directed towards a client‑centered environment, contributes to quality, operational efficiency, professional, and program development. The focus of the Transition Navigator is to support clients in a safe initial transition home and to assist with remaining healthy at home, while also benefitting from supportive rehabilitative care. The Transition Navigator will provide support to the client/family and team members from intake through post‑discharge, with a shared goal of decreasing Alternate Level of Care (ALC) days, while also reducing unnecessary hospital readmissions.
