ResponsibilitiesAssesses patients and collects information from a variety of sources using observation, communication, data review and physical assessment.
Formulates nursing diagnoses and determines appropriate nursing interventions. Plans, performs and evaluates nursing interventions and care. Collaborates with patients and families/caregivers in developing and modifying the care plan. Establishes a therapeutic relationship, respects privacy and autonomy, and supports informed decision making.
Participates in care and discharge planning on admission, updates the care plan, and collaborates with the interprofessional care team to ensure comprehensive planning and delivery. Coordinates nursing care, sets priorities, assigns and delegates tasks to support staff, and makes referrals to the interprofessional team per established protocols. Collaborates with physicians, LPNs, Patient Care Coordinator, Clinical Nurse Educators, and other health professionals to address issues, adjust plans, and respond to changes in patient condition. T eaches patients and families about illness prevention, health restoration and maintenance; assesses learning needs and provides information and demonstrations to support self-care and discharge planning.
Plans and discusses referral information with patients and families; initiates referrals to appropriate health care/community agencies in collaboration with the interprofessional team. Documents assessments, diagnoses, observations, interventions, patient responses, outcomes, and referrals per practice standards and Fraser Health procedures. Participates in nursing committees, care conferences and councils as assigned; contributes to quality improvement and research activities as appropriate. Attends in-services and other educational programs; maintains current clinical competence within area of practice.
Acts as a preceptor or mentor to new nursing staff and students. Provides feedback to the Manager regarding effectiveness of patient care programs, policy/clinical decision support tools, and equipment; performs other related duties as required. QualificationsEducation, Training & ExperienceGraduation from an approved School of Nursing with current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM). Cardiology component of Post Basic Critical Care or post-basic High Acuity Course, approved Arrhythmia interpretation course, plus one year of recent experience in cardiac monitoring or telemetry, or an equivalent combination of education, training and experience.
Current Basic Rescuer Certificate.

And AbilitiesClinical proficiency in the designated clinical area. Ability to assess patients and provide nursing care with clinical and diagnostic reasoning and safe interventions. Effective communication with patients, families, the public, medical staff and the interprofessional care team. Ability to promote person-centered care with empathy, cultural sensitivity and patient advocacy.
Ability to synthesize data from multiple sources to solve problems; teach patients and families about essential health topics. Organization and prioritization of work; collaboration with interprofessional team. Use of evidence-based practice to support nursing decisions; operation of related equipment and computer applications. Physical ability to perform duties as required.
