We have a Regular Full-Time opportunity for a Clinical Pharmacist to join our team at Lions Gate Hospital, located in North Vancouver, BC.ResponsibilitiesConsult with physicians, nurses and other health care providers to develop and discuss therapeutic plans appropriate for each patient; ensure documentation is maintained in the patient health record.Individualize patient medication therapy by reviewing charts, lab results, and patient interactions; modify or develop therapeutic plans as needed.Provide patient medication counselling and educate patients and families on drug therapy.Assist in developing drug therapy guidelines for individual disease conditions; participate in medication use evaluations and quality improvement initiatives; contribute to clinical drug trials when applicable.Serve as department resource within the assigned area and mentor pharmacy students, residents and new staff.Validate prescriptions, resolve medication safety concerns, and document professional practice activities.What you will need to joinCurrent registration as a Pharmacist with the College of Pharmacists of British Columbia.Completion of an accredited Hospital Pharmacy Residency Program or an equivalent combination of education, training and experience.Skills and AbilitiesBroad knowledge of organization policies, procedures and standards of care; knowledge of evidence-based clinical pharmacy practice.Knowledge of medication use evaluation and research processes; ability to apply clinical pharmacy knowledge with minimal supervision.Effective verbal and written communication; ability to establish and maintain working relationships with health practitioners, patients and families.Strong organizational and prioritization skills; ability to operate related equipment and manage dispensing processes.About Fraser HealthFraser Health is the heart of health care for over two million people in Metro Vancouver and the Fraser Valley in British Columbia.
