By working at The Care Company, you will be part of a growing organization that is helping individuals and families thrive at home.The Care Company is seeking a skilled and dedicated Physiotherapist (PT) with strong home and community care experience to join our growing team. This position is ideal for therapists who value flexibility, autonomy, and meaningful client relationships within a community-based model. Please note, this position will require in-person home visits with vulnerable community members.While we thank all interested applicants, we are only able to contact those who we are inviting to an interview.Key ResponsibilitiesProvide follow-up sessions focused on mobility, gait training, strength/balance retraining, pain management, and functional progressionDevelop, implement, and adjust individualized treatment plans in alignment with client goalsEducate clients and caregivers on home exercise programs, safety strategies, functional mobility, and equipment useIdentify and address fall risks, environmental barriers, and safety issues in the homeCollaborate with OTs, PSWs, nurses, physicians, and HCCSS/OH@H care coordinators to support coordinated careComplete documentation and charting accurately and in a timely manner according to community-care standardsMaintain ongoing communication with the office regarding availability, scheduling updates, and urgent visit capacityUphold The Care Company's commitment to safe, ethical, and compassionate community careSchedulingFlexible full‑time and part‑time (Monday‑Friday), occasional Saturday/Sunday availability based on client needs. Clients are located across Toronto, York, or Simcoe including but not limited to: Alliston, Bradford, Georgina, Newmarket, Aurora, Maple, Stouffville, Markham. Ability to independently manage scheduling, respond promptly to referrals, and coordinate visit times directly with clients. Travel may be required between client homes.Qualification and Experience RequirementsHome and community care experience is a MUST. Applicants without community experience will not be considered.Minimum 2 years of physiotherapy experience in home and community care, preferably with HCCSS/OH@H clients.Registered and in good standing with the College of Physiotherapists of Ontario.Demonstrated experience in mobility assessments, gait training, falls management, equipment recommendations, and post‑discharge rehab.Comfortable working independently in the community and making sound clinical decisions.Strong communication and documentation skills.Ability to travel throughout Toronto and the GTA for client visits.Valid driver's license and reliable transportation OR ability to navigate the GTA efficiently via transit.BenefitsCompensation$60‑$70 per visit, depending on experience and travel requirements.Other BenefitsFlexible scheduling to support work‑life balance.Opportunity to build a caseload in your area.Supportive administrative and clinical leadership team.Work in a culture that values autonomy, clinical excellence, and client‑centered care.Opportunities for clinical growth and expanded practice as our programs grow.Meaningful, relationship‑based care delivered in the comfort of clients' homes.#OTPT #J-18808-Ljbffr
