Clinical Team Lead- Home Visiting Service The Clinical Team Lead is a senior clinical role within the service, providing professional leadership, supervision, and support to the Home Visiting Service clinical team. The post-holder will work closely with the Clinical Directors to ensure the safe, effective and consistent delivery of the service. The role would be suitable for an advanced nurse practitioner, senior practice nurse, consultant nurse, senior paramedic, or advanced clinical practitioner. The candidate should have line management experience. Main duties of the job Clinical Leadership and Support Provide line management and clinical guidance to Home Visiting Service clinicians, supporting complex decision‑making and patient management. Support coordinators and clinicians with clinical triage and referrals, ensuring appropriate patient selection and escalation. Provide triage cover when required to maintain service continuity. Review and appropriately challenge referrals made outside the HVS inclusion criteria, ensuring safe, appropriate patient pathways. Offer advice and support to coordinators when uncertain about accepting referrals. Provide case‑based learning and feedback following complex cases, incidents or audit findings. Support the integration of clinical governance principles into daily practice. Participate in and lead clinical audits to ensure safety, quality and effectiveness of the HVS. Provide clinical supervision and mentoring for both substantive and locum clinicians. Identify training needs and develop learning events to support professional development across the team. Collaborate with Clinical, Operations and Educational Leads to align clinical delivery with operational processes. Promote professional development within the paramedic team, leading on clinical learning, audit and best practice. Contribute to strategic service development and innovation across the Sutton PCNs in partnership with Clinical Directors and Board Directors of Sutton PCNs. Clinical Practice Assess, diagnose and manage patients at home or in primary care settings, using advanced clinical reasoning and evidence‑based practice. Provide continuity of care through follow‑up and safety‑netting where required. Prescribe or recommend medication where appropriate and within scope of practice. Contribute to reducing unnecessary hospital admissions and improving patient outcomes. Maintain high‑quality clinical documentation and data integrity. Administrative and Governance Support Line‑manage the Home Visiting Service clinical team. Oversee completion and accuracy of end‑of‑day timesheets, task lists and activity reporting. Support clinical audit, data collection and reporting requirements. Contribute to service reviews, quality improvement projects and CQC readiness. Uphold information governance, confidentiality and data protection standards. About us Sutton has a population of approximately 200,000 residents registered across 21 GP practices within four Primary Care Networks (Carshalton, Cheam & South Sutton, Central Sutton, and Wallington). Each PCN serves a population of approximately 50,000 patients. Sutton PCNs work collaboratively to deliver high‑quality, innovative healthcare and to strengthen resilience across general practice. The Home Visiting Service (HVS) plays a vital role in supporting practices by managing acute presentations and providing continuity of care for frail and housebound patients, helping to reduce unnecessary hospital admissions. Person Specification Experience Significant post‑registration experience at least 5 years in primary, urgent, or community care. Experience working within a Primary Care Network or multi‑professional team. Experience supporting service transformation or clinical innovation. Qualifications Registered healthcare professional with current UK registration (e.g., NMC, HCPC, or equivalent). Significant post‑registration experience (normally 5 years or more) in primary care, urgent care, community care, or a relevant setting. Experience of clinical leadership and line management. Demonstrated ability to practise autonomously and manage complex clinical presentations. Strong communication, leadership, and problem‑solving skills. Sound understanding of clinical governance, audit, and quality improvement. Ability to work across sites, including undertaking home visits. Full UK driving licence and access to a vehicle. Advanced Clinical Practice qualification or working towards one. Independent prescribing qualification (or working towards). Experience working within a Primary Care Network or multidisciplinary team. Understanding of population health management and integrated care. Disclosure and Barring Service Check This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
