The organization values delivering high-quality patient care and supporting the health and well-being of patients, families and communities.Location: Langley, British ColumbiaSalaryThe salary range for this position is CAD $49.20 - $67.08 / hour.Job SummaryFraser Health coordinates hospice palliative care by assessing clients, coordinating care across the continuum, and acting as a clinical resource to the nursing and interdisciplinary team. The role includes providing clinical guidance, education, and quality improvement to ensure delivery of evidence-based patient care and alignment with care plans.ResponsibilitiesSupports the interdisciplinary team by monitoring standards of care and acting as a clinical nurse resource; assesses, screens and prioritizes hospice palliative referrals; role models and mentors the team in therapeutic communication with clients, families and caregivers.Coordinates and prioritizes client care activities by providing formal clinical consultations to nurses, team members and students; reviews client progress and adapts care plans to meet expected clinical outcomes; discusses referral options with staff to meet clinical needs.Consults with the interdisciplinary team and other healthcare providers to resolve client care issues; supports critical thinking, evaluates variances from guidelines, and collaborates with clients and families to minimize variances.Develops and implements clinical education, training and orientation plans for the interdisciplinary team; measures learning outcomes and applies teaching strategies accordingly.Assesses clinical skills by reviewing competencies and quality indicators; designs and evaluates education programs in hospice palliative care; conducts training for new team members and ongoing education for staff.Acts as a preceptor and mentor, providing evidence-based teaching and identifying skill deficiencies; coordinates with management on training needs and ensures staff demonstrate required competencies.Participates in quality improvement and risk management activities; evaluates changes to practice, completes safety audits, and recommends policy or procedure changes as needed.Participates in interdisciplinary rounds and care conferences; advocates for the client’s dignity, autonomy, privacy and access to information.Assists in staff selection and performance evaluation; contributes to orientation and ongoing performance discussions and follow-up actions as needed.Promotes the use of research in nursing and keeps program content current by integrating evidence into practice.Sets measurable goals and maintains clinical competence through professional development with management collaboration; monitors progress to ensure timely achievement of goals.Performs other related duties as assigned.QualificationsEducation and ExperienceCurrent practicing registration as a Registered Nurse and/or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM). Three (3) years of recent related clinical nursing experience in the designated clinical area, including one (1) year in a supervisory/leadership role, or an equivalent combination of education, training and experience. Completion of applicable Canadian Nursing Association (CNA) certificate in the designated clinical area.Valid BC Driver's License and access to a personal vehicle for business-related purposes.Skills And AbilitiesComprehensive knowledge of nursing theory and evidence-based hospice palliative best practices.Knowledge of adult learning principles and critical thinking.Ability to develop, implement and revise education/training programs.Ability to provide leadership through modeling, coaching and mentoring.Ability to instruct others and facilitate learning of staff and clients/families.Strong communication skills, both verbal and written, with clients, families and colleagues.Ability to work independently and as part of an interdisciplinary team.Ability to build rapport with staff, clients and stakeholders.Problem-solving and conflict management abilities.Ability to plan, organize and prioritize in a changing environment.Ability to operate related software and equipment.Physical ability to perform the duties of the position.About Fraser HealthFraser Health serves over two million people in Metro Vancouver and the Fraser Valley on the traditional lands of the Coast Salish and Nlaka’pamux Nations.
